摘要:
目的 探讨溴甲酚绿(bromocresol green, BCG)法和溴甲酚紫(bromocresol purple, BCP)法测定血清清蛋白 (albumin, Alb)对糖化清蛋白(glycated albumin, GA) 的结果影响。方法 回顾性收集2018 年1~12 月于西安交通大学 第一附属医院就诊患者溴甲酚绿法清蛋白(AlbBCG)和溴甲酚紫法清蛋白(AlbBCP)的检验数据,分别计算溴甲酚绿法 糖化清蛋白(GABCG)和溴甲酚紫法糖化清蛋白(GABCP),并比较两种方法所测得的结果是否存在差异。结果 随着 患者体内Alb 水平的降低,两种方法测得的Alb 值和GA 值出现显著性差异。当Alb<35g/L 时,BCG 法比BCP 法Alb 测定的Alb 值略高,约高2.18g/L;BCG 法比BCP 法测定的GA 值略低,约低1.01%。当Alb ≥ 35g/L 时, 两种方法测 定GA 值无显著统计学差异(t=2.0,P=0.05)。两种检测方法计算GA 结果一致性均较好,但当Alb ≥ 35g/L 时, 两种 方法对于GA 检测一致性更高。结论 在糖化清蛋白检测中,若患者血清中Alb 较低,BCG 法非特异性反应增加,导 致所测的GA 值低于BCP 法所测得的GA 值,可能低估患者GA 水平。因此,BCP 法更适合检测伴有低蛋白血症糖尿 病患者的GA 值。
Abstract:
Objective To compare bromocresol green (BCG) method and bromocresol purple (BCP) method for calculation of glycated albumin (glycated albumin, GA) by detecting serum albumin (Alb). Methods The data from the First Affiliated Hospital of Xi’an Jiaotong University from January to December 2018 were retrospectively collected and statistically analyzed. Results At the low level of the Alb, significant inconsistencies were observed between the two methods for detection of Alb and GA. When Alb<35g/L, Alb detected by BCG method (AlbBCG)was about 2.18 g/L higher than that detected by BCP method (AlbBCP), and the corresponding GA values detected by BCG method (GABCG) were about 1.01% lower than that detected by BCP (GABCP) .When Alb>35g/L, there was no significant difference in GA values detected by the two methods (t=2.0,P > 0.05). Alb values detected by the two methods showed high consistency, however, the two methods had higher consistency for GA detection when Alb ≥ 35g/L.Conclusion For the detection of glycated albumin, GABCG might be lower than the actual GA level in patients when Alb levels are low in patients, so recommend BCP method for detection of GA in diabetics with hypoproteinemia.
