摘要:
目的 探讨急性心肌梗死(acute myocardial infarction,AMI)患者血浆外泌体miR-21-3p/PTEN/AKT 表达水平及诊断价值。方法 选取2019 年2 月~ 2021 年8 月于陕西省汉中市三二〇一医院内分泌与风湿免疫科住院并诊断为急性心肌梗死的120 例患者作为AMI 组,另选同期在该科住院,经冠脉造影发现冠脉无明显狭窄患者120 例为对照组。提取所有研究对象的血浆外泌体,并采用透射电镜和Western blot 鉴定。qRT-PCR 法检测外泌体中miR-21-3p 表达水平,Western blot 检测外泌体中蛋白酪氨酸酶基因(PTEN)和丝苏氨酸激酶(AKT)蛋白的表达水平。Pearson 相关分析明确AMI 患者血浆外泌体中miR-21-3p,PTEN 与AKT 表达水平之间的相关性。绘制受试者工作特征(receiver operatorcharacteristic curve,ROC)曲线,根据曲线下面积(area under curve, AUC)分析血浆外泌体miR-21-3p,PTEN 及AKT对AMI 的联合诊断价值。结果 透射电镜观察到,血浆外泌体呈双层膜的泡状结构,且外泌体特征蛋白CD9,CD63和CD81 在外泌体中表达明显升高。与对照组相比,AMI 患者的miR-21-3p(1.45±0.31 vs 1.05±0. 17)和AKT(1.23±0.27 vs 1.03±0.15)表达显著上升,而PTEN(0.68±0.10 vs 1.01±0. 23 )表达水平显著下降,差异有统计学意义(t=125.735,112.628,72.309,均P < 0.001)。Pearson 相关性分析显示,AMI 患者血浆外泌体中miR-21-3p 和PTEN 呈明显负相关(r=-0.440,P=0.000),PTEN 和AKT 呈明显负相关(r=-0.674,P =0.000),而miR-21-3p 和AKT 之间无明显相关性(r=-0.072,P=0.438)。ROC 曲线结果显示,血浆外泌体miR-21-3p,PTEN 及AKT 单独检测时,其对应曲线下面积(AUC)分别为0.805,0.782 及0.720,联合检测时AUC 为0.885,比单独检测更具有诊断价值,差异有统计学意义(P<0.001)。结论 AMI 患者血浆外泌体中miR-21-3p 表达上升,其与PTEN 呈负相关,PTEN 又负向调控AKT 表达。三项指标均可作为辅助诊断AMI 的潜在标志物,且联合检测对AMI 有较高的诊断价值。
Abstract:
Objective To investigate the expression level and diagnostic value of plasma exosomal miR-21-3p/PTEN/AKT in patients with acute myocardial infarction(AMI). Methods 120 patients diagnosed with AMI in the Deparment of Endocrinology and Rheumatolgy 3201 Hospital in Hanzhong City from February 2019 to August 2021 were selected as the AMI group, and 120 patients who were hospitalized in the department during the same period and found no obvious coronary stenosis by coronary angiography were selected as control group. Plasma exosomes from all subjects were extracted and identified by transmission electron microscopy and Western blot. The expression levels of miR-21-3p in exosomes were detected by qRT-PCR, and the expression levels of PTEN and AKT proteins in exosomes were detected by Western blot. Pearson correlation analysis clarifies the correlation between the expression levels of miR-21-3p, PTEN and AKT in plasma exosomes from AMI patients. The ROC curve was drawn, and the combined diagnostic value of plasma exosomal miR-21-3p, PTEN and AKT for AMI was analyzed according to the area under the curve AUC. Results Transmission electron microscopy showed that plasma exosomes had a double-membrane vesicle structure, and the expression of exosome characteristic proteins CD9, CD63 and CD81 was significantly increased in exosomes. Compared with the control group, the expression of miR-21-3p(1.45±0.31 vs 1.05±0. 17)and AKT(1.23±0. 27 vs 1.03±0.15 )in AMI patients increased significantly, while the expression of PTEN(0.68±0.10 vs 1.01±0. 23 )decreased significantly, and the differences were statistically significant (t=125.735, 112.628, 72.309, all P < 0.001). Pearson correlation analysis showed that miR-21-3p and PTEN in serum exosomes of AMI patients were significantly negatively correlated, PTEN and AKT were significantly negatively correlated, and there was no significant correlation between miR-21-3p and AKT. The ROC curve results showed that when serum exosomes miR-21-3p, PTEN and AKT were detected separately, the corresponding area under the curve AUC were 0.805, 0.782 and 0.720, respectively, and the combined detection AUC was 0.885, which was more diagnostic than the single detection (P<0.001). Conclusion The expression of miR-21-3p in plasma exosomes of AMI patients was increased, which was negatively correlated with PTEN, which in turn negatively regulated the expression of AKT. All three indicators can be used as potential markers for auxiliary diagnosis of AMI, and combined detection has high diagnostic value for AMI.
