目的 探讨骨髓增生异常综合征(myelodysplastic syndrome, MDS)患者外周血单个核细胞中八聚体结合转录因子4(octamer binding transcription factror 4, OTC4),Gli1 基因的表达及与预后的COX 回归分析。方法 对2017 年3 月~2018 年3 月收治的54 例MDS 患者进行临床及随访研究,另选取40 例同期体检健康者为对照组,采集所有受试者骨髓后分离单个核细胞,以实时荧光定量PCR 法(RT-PCR)检测OTC4 和Gli1 基因相对表达情况,并分析其与MDS 患者临床病理特征的关系。采用Kaplan-Meier 法绘制MDS 患者生存曲线,并采用COX 回归分析MDS 患者预后的影响因素。结果 OTC4(2.78±0.31),Gli1(1.35±0.33)基因在MDS 患者外周血单个核细胞中的相对表达水平明显高于健康对照组(1.07±0.17,0.53±0.13),差异有统计学意义(t=31.524,14.868,均P<0.05)。OTC4 基因表达与MDS 患者Hb,PLT,IPSS-R 危险分层有关;Gli1 基因表达与MDS 患者PLT,IPSS-R 危险分层有关(t=2.722, 2.282,均P<0.05),而与Hb 无关(t=0.711,P>0.05)。OTC4 基因低表达MDS 患者OS(36.73±1.46 个月),PFS(26.28±0.91个月)均明显长于OTC4 基因高表达MDS 患者水平(29.10±1.26 个月,22.54±1.08 个月)(Log-rank χ2=10.038,13.623,均P<0.05);Gli1 基因低表达MDS 患者OS(35.74±1.29 个月),PFS(27.81±0.94 个月)均明显长于OTC4基因高表达MDS 患者水平(26.16±1.37 个月,19.61±0.96 个月)(Log-rank χ2=16.210,20.135,均P<0.05)。COX多因素回归分析显示,WBC,WHO(2008)分型、IPSS-R 危险分层、OTC4 基因相对表达水平、Gli1 基因相对表达水平均为阳性MDS 患者预后的独立影响因素(P<0.05)。结论 OTC4,Gli1 基因在MDS 患者外周血单个核细胞中呈高表达,且与WBC,WHO(2008)分型、IPSS-R 危险分层均为影响MDS 患者预后的独立因素。抑制OTC4,Gli1 基因表达有望成为MDS 潜在治疗靶点。
Abstract:
Objective To explore the expression of octamerbinding transcription factor 4(OTC4) and Gli1 genes in peripheral blood mononuclear cells of patients with myelodysplastic syndrome(MDS) and their correlation with prognosis by COX regression. Methods A clinical and follow-up study was conducted on 54 MDS patients who were treated from March 2017 to March 2018, another 40 healthy subjects who underwent physical examination during the same period were selected as the control group. The bone marrow of all subjects was collected and the mononuclear cells were isolated and quantified by real-time fluorescence PCR method (RT-PCR) was used to detect the relative expression of OTC4 and Gli1 genes, and to analyze their relationship with clinicopathological characteristics of MDS patients. The Kaplan-Meier method was used to draw the survival curve of MDS patients, and COX regression was used to analyze the influencing factors of the prognosis of MDS patients. Results The relative expression levels of OTC4 (2.78±0.31) and Gli1 (1.35±0.33) genes in peripheral blood mononuclear cells of MDS patients were significantly higher than those in healthy controls(1.07±0.17, 0.53±0.13), the differences were statistically significant (t=31.524, 14.868, all P<0.05). OTC4 gene expression was related to Hb, PLT, IPSS-R risk stratification in MDS patients(t=4.145, 2.085, 2.430, all P < 0.05); Gli1 gene expression was related to PLT, IPSS-R risk stratification in MDS patients (t=2.722, 2.282, all P<0.05), but not Hb (t=0.711, P>0.05). The OS (36.73±1.46 ) months and PFS (26.28±0.91) months of MDS patients with low OTC4 gene expression were significantly longer than those of MDS patients with high OTC4 gene expression (29.10±1.26, 22.54±1.08)months (Log-rank χ2=10.038, 13.623, all P<0.05). OS (35.74±1.29) months and PFS (27.81±0.94 ) months of MDS patients with low Gli1 gene expression were significantly longer than those of OTC4, the level of high gene expression in MDS patients (26.16±1.37, 19.61±0.96) months(Log-rank χ2=16.210, 20.135, all P<0.05). COX multivariate regression analysis showed that WBC, WHO (2008) classification, IPSS-R risk stratification, relative expression level of OTC4 gene and relative expression level of Gli1 gene were independent prognostic factors for positive MDS patients (P<0.05). Conclusion OTC4 and Gli1 genes are highly expressed in peripheral blood mononuclear cells of MDS patients, and are independent factors affecting the prognosis of MDS patients with WBC, WHO (2008) classification and IPSS-R risk stratification. Inhibition of OTC4, Gli1 gene expression is expected to become a potential therapeutic target for MDS.
