摘要:
目的 探讨原发性高血压患者醛固酮肾素比值(aldosterone-renin ratio ,ARR),C1q/肿瘤坏死因子相关蛋白 9(C1q/tumor necrosis factor-related protein 9,CTRP9)和 miR-663表达水平与靶器官损害的相关性。方法 选择 2020年 5月~2022年 5月鄂东医疗集团黄石市中心医院收治的 162例原发性高血压患者为研究对象,根据靶器官损害状况分为复合损害组、单一损害组和无损害组。比较三组一般资料、 ARR,CTRP9和 miR-663水平。应用 Spearman相关性分析观察 ARR,CTRP9和 miR-663水平与原发性高血压患者靶器官损害的关系。绘制受试者工作特征(ROC)曲线分析 ARR,CTRP9和 miR-663对原发性高血压患者出现单一、复合靶器官损害的预测效能。结果 复合损害组 ARR水平(18.26±2.29)高于单一损害组(15.06±2.47)、无损害组(13.59±2.60),差异有统计学意义(F= 50.799,P= 0.000);单一损害组 ARR水平高于无损害组,差异有统计学意义(t= 2.768,P= 0.007)。复合损害组 CTRP9(112.45±22.57 ng/ml)和 miR-663(0.42±0.15)水平均低于单一损害组(125.37±24.45 ng/ml,0.56±0.18)、无损害组(145.84±27.38 ng/ml,0.71±0.20),差异有统计学意义(F= 21.861,33.514,均 P= 0.000);单一损害组 CTRP9, miR-663水平均低于无损害组 ,差异有统计学意义(t= 3.796,3.971,均 P= 0.000)。Spearman相关性分析发现, ARR水平与原发性高血压患者靶器官损害呈正相关(r= 0.706, P= 0.000),CTRP9和 miR-663水平与原发性高血压患者靶器官损害呈负相关(r= -0.593,-0.641,均 P= 0.000);绘制 ROC曲线分析显示, ARR,CTRP9和 miR-663联合预测原发性高血压患者出现单一、复合靶器官损害的曲线下面积(AUC)分别为 0.815,0.970,均高于单一指标预测。结论 原发性高血压患者 ARR, CTRP9和 miR-663表达水平与靶器官损害密切相关,联合检测可为患者靶器官损害预测提供一定参考依据。
Abstract:
Objective To investigate the correlation between aldosterone-renin ratio (ARR), C1q/tumor necrosis factor-related protein 9 (CTRP9) and miR-663 expression and target organ damage in patients with essential hypertension. Methods 162 patients with essential hypertension who were admitted to Huangshi Central Hospital of Edong Medical Group from May 2020 to May 2022 were selected as the study subjects, and they were divided into compound damage group, single damage group, no damage group. The general data of the three groups were compared, and the levels of ARR, CTRP9 and miR-663 were detected and compared in the three groups. Spearman correlation analysis was used to observe the relationship between ARR, CTRP9, miR-663 levels and target organ damage in patients with essential hypertension. Receiver operating characteristic (ROC) curve analysis of the predictive power of ARR, CTRP9 and miR-663 for single and compound target organ damage in patients with essential hypertension. Results The ARR levels of the compound damage group (18.26±2.29) were higher than those of the single damage group(15.06±2.47)and the no damage group(13.59±2.60), and the differences were statistically significant (F=50.799, P=0.000). The ARR level of the single damage group was higher than that of the no damage group, the difference was statistically significant (t=2.768, P=0.007). The levels of CTRP9 (112.45±22.57 ng/ml ) and miR-663 (0.42±0.15) in the compound damage group were lower than those in the single damage group(125.37±24.45 ng/ml,0.56±0.18)and the no damage group(145.84±27.38 ng/ml,0.71±0.20), and the differences were statistically significant (F=21.861,33.514,all P=0.000), and the levels of CTRP9 and miR-663 in the single damage group were lower than those in the no damage group, the differences were statistically significant (t=3.796, 3.971, all P=0.000). Spearman correlation analysis found that ARR levels were positively correlated with target organ damage in patients with essential hypertension(r= 0.706, P= 0.000), while CTRP9 and miR-663 levels were negatively correlated with target organ damage in patients with essential hypertension(r=-0.593,-0.641,all P= 0.000). The ROC curve analysis showed that the area under the curve (AUC) of ARR, CTRP9 and miR-663 combined to predict single and compound target organ damage in patients with essential hypertension were 0.815 and 0.970, respectively, which were higher than those predicted by a single index. Conclusion The expression levels of ARR, CTRP9 and miR-663 in patients with essential hypertension were closely related to target organ damage, and the combined detection can provide a certain reference for the prediction of target organ damage in patients.
