摘要:
目的 探究血清miR-135b 和miR-145 在未破裂颅内动脉瘤(unruptured intracranial aneurysm,UIA)患者中的表达水平及临床价值。方法 选取2018 年5 月~ 2020 年3 月邯郸市第一医院神外三科收治的102 例UIA 患者作为研究组,另选同期健康体检者95 例作为对照组,同时收集受试者一般资料进行比较。实时荧光聚合酶链反应(qRT-PCR)法检测血清中miR-135b 和miR-145 表达水平;ROC 曲线分析血清miR-135b 和miR-145 对UIA 的诊断价值;Logistic回归分析UIA 形成的影响因素。结果 研究组中高血压(62.75%)、高血脂(60.00%)、吸烟史(64.76% )、饮酒史(70.59%)患者所占比例高于对照组(46.72%,44.21%,47.37%,53.68%),差异均有统计学意义(χ2=5.361,4.986,6.139,5.993,均P < 0.05);研究组患者血清miR-135b(0.79±0.20)表达水平低于对照组(1.03±0.27),血清miR-145 水平(1.28±0.24)高于对照组(0.99±0.22),差异均有统计学意义(t=7.122,8.821,均P < 0.05);与动脉瘤个数为1 及动脉瘤直径<7mm 的患者相比,动脉瘤个数≥ 2 及动脉瘤直径≥ 7mm 的患者血清中miR-135b 表达水平(0.68±0.18 vs 0.84±0.21,0.71±0.17mm vs 0.85±0.22mm)降低,miR-145 水平(1.37±0.31mm vs 1.24±0.21mm,1.37±0.30mm vs 1.21±0.19mm)升高,差异均有统计学意义(t=2.472 ~ 3.689,均P < 0.05);血清miR-135b,miR-145 以及二者联合诊断UIA 的曲线下面积(AUC)分别为0.788,0.836 和0.907,二者联合的诊断效能优于其各自单独检测(Z=4.236,3.226,均P < 0.05);Logistic 回归分析结果显示,高血压(OR=1.326, 95%CI=1.044 ~ 1.684)、吸烟史(OR=1.457, 95%CI=1.077 ~ 1.970)、饮酒史(OR=1.879, 95%CI=1.113 ~ 3.171)、血清miR-135b 水平<0.94(OR=0.758, 95%CI=0.612 ~ 0.939)、血清miR-145 水平≥ 1.22(OR=1.433, 95%CI=1.113 ~ 1.845)均是UIA 的影响因素(均P<0.05)。结论 UIA 患者血清miR-135b 水平下调,miR-145 水平上调,二者与UIA 的形成和发展存在相关性,且二者联合检测对UIA 形成的诊断效能较高。
Abstract:
Objective To investigate the expression and clinical significance of serum miR-135b and miR-145 in patients with unruptured intracranial aneurysm (UIA). Methods A total of 102 patients with UIA admitted in the First Hospital of Handan City from May 2018 to March 2020 were selected as the study group, and 95 healthy people who participated in physical examination in First Hospital of Handan City during the same period were selected as the control group. At the same time, general data of the subjects were collected for comparison. Serum miR-135b and miR-145 levels were detected by real-time fluorescence PCR(RT-PCR). ROC curve was used to analyze the diagnostic value of serum miR-135b and miR-145 for UIA, and Logistic regression was used to analyze the influencing factors of UIA formation. Results The proportions of patients with hypertension(62.75%), hyperlipidemia(60.00%), smokinghistory(64.76%) and drinking history (70.59%) in the study group were obviously higher than those in the control group (46.72%, 44.21%, 47.37%, 53.68%),the differences were statistically significant(χ2=5.361,4.986,6.139,5.993,all P<0.05). The expression level of serum miR-135b(0.79±0.20) in the study group was obviously lower than that in the control group(1.03±0.27),the level of serum miR-145(1.28±0.24)was obviously higher than that in the control group (0.99±0.22),the differences were statistically significant(t=7.122,8.821,all P < 0.05). Compared with patients with aneurysm number of 1 and aneurysm diameter <7 mm, the expression level of miR- 135b in serum of patients with aneurysm number ≥2 and aneurysm diameter ≥7 mm(0.68±0.18 vs 0.84±0.21; 0.71±0.17mm vs 0.85±0.22mm) decreased obviously, and the level of miR-145 (1.37±0.31mm vs 1.24±0.21mm; 1.37±0.30mm vs 1.21±0.19mm)increased obviously, ,the differences were statistically significant (t=2.472 ~ 3.689, all P<0.05). The area under the curve (AUC) of serum miR-135b, miR-145 and their combination in the diagnosis of UIA was 0.788, 0.836 and 0.907, respectively, the diagnostic efficacy of the combination of miR-135b and miR-145 was obviously better than that of their separate detection (Z=4.236,3.226,all P<0.05). Logistic regression analysis showed that hypertension(OR=1.326, 95%CI=1.044~1.684), smoking history(OR=1.457, 95%CI=1.077~1.970), drinking history(OR=1.879, 95%CI=1.113 ~ 3.171), serum miR-135b level <0.94(OR=0.758, 95%CI=0.612 ~ 0.939) and serum miR-145 level ≥ 1.22(OR=1.433, 95%CI=1.113 ~ 1.845) were all influencing factors of UIA (all P<0.05). Conclusion The level of serum miR-135b in patients with UIA was down-regulated, and the level of miR-145 is up-regulated. Both of them were related to the formation and development of UIA, and the combination of them has a high diagnostic efficiency for UIA.
