摘要:
目的检测尿液外泌体miR-23b-3p在儿童原发性肾病综合征(nephrotic syndrome,NS)患者中的水平,评估其作为NS辅助诊断指标的临床价值。 方法收集2017年3~11月在解放军东部战区总医院住院并确诊的98例NS患儿(NS组)和南京医科大学附属儿童医院95例年龄、性别相匹配的健康儿童(对照组)的晨尿标 本及临床资料。实时荧光定量PCR (quantitative real-time PCR,qRT-PCR)方法检测miR -23b-3p在尿液外泌体中的含量。应用受试者工作特征曲线(receiver operating charact eristic curve,ROC)、相关性分析等评估其作为儿童NS诊断指标的临床价值。 结果miR-23b-3p在NS组尿液外泌体中的浓度明显高于对照组[13.44(4.47,31.40)fmol/L vs 3.94(2.76,7.15)fmol/L,差异有统计学意义, U=1 899,P <0 .000 1],且在治疗缓解后显著下降[10.62(4.38,37.72)fmol/L vs 2.84(1.53,4 .92)fmol/L, W=695,P <0.000 1]。miR-23b-3p诊断NS的ROC曲线下面积(AUC)为0.796(95%CI:0.734~0.858),敏感度为84.7%,特异度为64.2%。相关性分析显示,NS患儿尿液外泌体miR-23b-3p与血清总蛋白( r=-0.377)、清蛋白(r =-0.309)水平呈显著负相关,与血清总胆固醇( r=0.266)、三酰甘油(r =0.318)呈显著正相关,差异均具有统计学意义(均 P <0.01)。结论miR-23b-3p在NS患儿尿液外泌体中的含量显著升高,且在治疗缓解后明显降低,有望作为儿童NS 辅助诊断和治疗效果监测的潜在指标。
Abstract:
Objective To investigate the levels of urinary exosomal miR-23b-3p in children with nephrotic syndrome (NS) and evaluate its clinical significance as a non-invasive auxiliary diagnostic indicator.Methods Urinary samples were collected from 98 children with NS and 95 age-and gender-matched healthy controls in Eastern Theater General Hosp ital of PLA and Children’s Hospital of Nanjing Medical University from March to November 2017,respectively.The concentrations of urinary exosomal miR-23b-3 p were measured by quantitative real-time polymerase chain reaction (qRT-PCR) in NS and control children.The receiver operating characteristic (ROC) curves a nd correlation analysis were conducted to evaluate the diagnostic value of urina ry exosomal miR-23b-3p for children with NS.Results Ur inary exosomal miR-23b-3p was significantly evaluated in NS group as compared with control group [13.44 (4.47,31.40) fmol/L vs 3.94 (2.76,7.15) fmol/ L],the difference was statistically significant ( U=1 899,P <0.000 1),and markedly declined in clinical remission period [10.62 (4.38,37.72) fmol/L vs 2.84 (1.53,4.92) fmol/L, W=695,P <0.000 1].The ROC analysis indica ted that the area under the curve (AUC) was 0.796 (95%CI:0.734~0.858) for th e diagnosis of NS children,with sensitivity of 84.7% and specificity of 64.2% .Spearman correlations analysis indicated that the concentration of the miR-23 b-3p was significantly negatively correlated with serum total protein ( r =- 0.377) and serum albumin ( r =-0.309),and was significantly positively cor related with serum total cholesterol ( r =0.266) and triacylglycerols ( r = 0.318) in NS children,all with statistical significance (all P <0.01).Conclusion The concentration of miR-23b-3p in urine exosom es was significantly elevated in children with NS,and was markedly reduced afte r treatment remission,suggesting that urinary exosomal miR-23b-3p may have th e potential to serve as an auxiliary diagnostic indicator for NS.
