摘要:
目的 探讨血清酸性鞘磷脂酶(acid sphingomyelinase,ASM) 和谷胱甘肽过氧化酶(glutathione peroxidase,GPx) 水平与类风湿性关节炎(rheumatoid arthritis,RA) 膝关节病变的相关性。方法 选择2016 年3 月~2019 年3 月于咸阳市第一人民医院、陕西省森工医院和咸阳市妇幼保健院就诊的58 例RA 膝关节病变患者作为研究对象。依据疾病活动评分28(disease activity score,DAS28) 将RA 患者分为缓解期组(25 例) 和活动期组(33 例),选择30 例同期体检健康者作为对照组。血清ASM 和GPx 水平检测采用酶联免疫吸附法,血清抗环瓜氨酸抗体( anti-cyclic peptidecontainingcitrulline,Anti-CCP) 和类风湿因子(rheumatoid factor,RF) 水平检测采用乳胶增强免疫比浊法。所有研究对象均接受膝关节超声检查,比较分析以上指标的变化与RA 的相关性。结果 对照组、缓解期组和活动期组的血清ASM 和GPx 水平分别为 21.35±6.61,72.30±13.49,123.67±19.26 nmol/L 和93.26±13.07, 65.13±11.29,41.07±8.65 U/ml。缓解期组和活动期组的GPx 水平与对照组比较显著降低, 活动期组的GPx 水平与缓解期组比较显著降低(F=68.23,P=0.000)。缓解期组和活动期组的ASM 水平与对照组比较显著增高, 活动期组的ASM 水平与缓解期组比较显著增高(F=79.15,P =0.000)。Pearson 线性相关分析显示, 缓解期组和活动期组的GPx 水平分别与RF,Anti-CCP 水平、髌上囊滑膜厚度及髌上囊积液深度呈负相关性(r 缓解期组=-0.832,-0.829,-0.813 和-0.809,P <0.01;r 活动期组=-0.851,-0.836,-0.827和-0.815,P<0.01)。缓解期组和活动期组的ASM 水平分别与RF,Anti-CCP 水平、髌上囊滑膜厚度和髌上囊积液深度呈正相关性(r 缓解期组=0.820,0.811,0.789 和0.793,P<0.01;r 活动期组=0.837,0.825,0.807 和0.817,P<0.01)。结论 ASM和GPx 与RA 的发生发展密切相关。检测血清ASM 和GPx 水平可用于评价RA 的病情进展并为RA 的诊治提供依据。
