摘要:
目的 探讨低色素性贫血对于正常妊娠期妇女的糖化血红蛋白水平的影响。方法 回顾性分析2017年1月~2018年12月来东莞康华医院建立生育档案的孕周为10~28周、空腹血糖(fasting blood-glucose; FBG)水平正常的妊娠期妇女618例,其中血红蛋白Hb≥110 g/L 337例和Hb<110 g/L 281例。选取低色素组作为观察组,即根据平均红细胞体积(MCV)及平均红细胞血红蛋白量(MCH)将妊娠期妇女分为小细胞低色素性贫血组73例、正细胞低色素性贫血组69例;选取正细胞正色素为对照组,其中正细胞正色素性贫血组139例,正细胞正色素性非贫血组337例。检测两组妊娠期妇女的血细胞检测、FBG和糖化血红蛋白,并结合结果进行比较和统计学分析。结果 根据分析的结果可知,贫血组与非贫血组的FBG水平分别为4.260±0.45mmol/L和4.351±0.57mmol/L,两者差异无统计学意义(t=0.255,P=0.135),糖化血红蛋白水平分别为4.993%±0.35% 和4.983%±0.33%,两者差异无统计学意义(t=0.361,P=0.718);小细胞低色素性贫血组、正细胞低色素贫血组、正细胞正色素性贫血组和正细胞正色素非贫血组这四组的FBG水平分别为4.253±0.32,4.294±0.37,4.247±0.61和4.351±0.57mmol/L,两两比较差异无统计学意义(F=1.603,P=0.187),糖化血红蛋白水平分别为5.007%±0.35%,5.093%±0.38%,4.936%±0.32%和4.983%±0.33%,两两比较差异有统计学意义(F=3.492,P=0.015),正细胞低色素性贫血组和正细胞正色素性贫血组比较差异有统计学意义(P=0.009),正细胞低色素性贫血组的糖化血红蛋白水平高于正细胞正色素性贫血组。结论 低色素性贫血会影响妊娠期妇女糖化血红蛋白水平,引起糖化血红蛋白水平增高。
