摘要:
目的 探讨系统性红斑狼疮(systemic lupus erythematosus,SLE)患者并发EB病毒(epstein barr virus,EBV)或巨细胞病毒(cytomegalovirus,CMV)感染的流行病学特征,为临床提供参考。方法 回顾性分析2012年1月～2015年4月在南京大学医学院附属鼓楼医院就诊入院的202例女性SLE患者(SLE组)的相关临床资料,以同期的女性肾移植供者和妇产科送检标本共203例作为对照组; 根据实时荧光定量PCR检测外周血中EB病毒DNA和巨细胞病毒DNA结果,分析、比较SLE组和对照组的感染发生率。结果 SLE患者中EBV-DNA的阳性率(11.39%)显著高于对照组的阳性率(3.45%),差异具有统计学意义(χ2=8.28,P<0.01); SLE组CMV-DNA的阳性率(7.92%)显著高于对照组的阳性率(1.97%),差异具有统计学意义(χ2=7.64,P<0.05)。128例育龄期SLE患者EBV-DNA的阳性率(10.94%)显著高于对照组的阳性率(3.45%),差异具有统计学意义(χ2=4.99,P<0.05); 育龄期SLE组CMV-DNA的阳性率(7.75%)显著高于对照组的阳性率(2.22%),差异具有统计学意义(χ2=4.31,P<0.05)。结论 SLE患者易并发EB病毒或巨细胞病毒感染,提示SLE与病毒感染可能具有相关性,在临床工作中应加以重视。
Abstract:
Objective To analyze the epidemiological characteristics of Epstein Barr virus(EBV)or cytomegalovirus(CMV)infection in systemic lupus erythematosus(SLE)patients in order to provide reference for clinic.Methods The clinical data of 202 female cases in-patients diagnosed with SLE(SLE group)from January 2012 to May 2015 in Nanjing DrumTower Hospital were analyzed retrospectively.Meanwhile,203 cases including female renal transplant donors,obstetrics and gynecology in-patients selected randomly were enrolled as control group.The infection rate between SLE and control groups was analyzed and compared based on the results of EBV-DNA and CMV-DNAin peripheral blood quantified by real time PCR.Results The positive rate of EBV-DNA in SLE group was 11.39%(23/202),with significantly statistical difference when comparing with the control group [3.45%(6/174)](χ2=8.28,P<0.01).The positive rate of CMV-DNA [7.92%(16/202)]was significantly higher than that in control group [1.97%(4/203)](χ2=7.64,P<0.05).In addition,the positive rate of EBV-DNA and CMV-DNA wasalso greatly higher in reproductive-age(20～45 years old)of SLE patients thanthose in control group,10.94%(14/128)vs 3.45%(4/116)(χ2=4.99,P<0.05)and 7.75%(10/129)vs 2.22%(3/135)(χ2=4.31,P<0.05),respectively.Conclusion SLE patients were more inclined to be accompanying infected by EBV or CMV,indicating the possible correlation between SLEand EBV or CMV infection; and physicians should pay more attention to the viralinfection of SLE in clinical treatment.
