摘要:
目的 探讨血清肿瘤标志物人附睾蛋白4(human epididymis, HE4)对肺癌的诊断价值。方法 以80例肺癌患者为病例组,30例肺部良性疾病患者为良性对照组,30例健康人为健康对照组,检测所有研究对象的血清癌胚抗原(CEA),细胞角蛋白片段21-1(CYFRA21-1),神经元特异性烯醇化酶(NSE)和HE4水平。结果 病例组血清CEA,NSE,CYFRA21-1和HE4水平均高于良性对照组或健康对照组(P<0.05)。四种标志物诊断肺癌患者的受试者工作特征曲线(ROC曲线)下面积(AUC)分别为0.870,0.818,0.746和0.897。腺癌患者血清CEA和HE4,鳞癌患者血清CYFRA21-1,小细胞肺癌(SCLC)患者血清NSE水平较高(P<0.05)。血清CYFRA21-1(AUC=1.000),CEA(AUC=0.727)和HE4(AUC=0.622)水平检测对鳞癌诊断有价值(P<0.05),血清CEA(AUC=0.954)和HE4(AUC=0.944)水平检测对腺癌诊断有价值(P<0.05),检测血清NSE(AUC=0.876)水平对SCLC诊断有价值(P<0.05)。结论 肺癌患者血清HE4,CEA,NSE和CYFRA21-1等肿瘤标志物表达异常,患者血清HE4水平与肿瘤的病理类型及转移情况相关,血清HE4对腺癌有较高的敏感度,血清HE4水平检测可用于肺癌辅助诊断及病情评估。
Abstract:
Objective To investigate the diagnostic valuesof human epididymis(HE4)in lung cancer.Methods 80 patients with lung cancer were the experimental group,30 patients with benign pulmonary disease were the benign control group,and 30 healthy people were healthycontrol group.The levels of carcinoma embryonic antigen(CEA),cytokeratin protein fragment 21-1(CYFRA21-1),neuron specific enolase(NSE)and HE4 in serumwere detected.Results The levels of CEA,NSE,CYFRA21-1 and HE4 in lung cancer patients were higher than those in both the benign control group and the healthy control group(P<0.05).The areas(AUC)under thereceiver operating characteristic(ROC curve)were 0.870,0.818,0.746 and 0.897 for serum CEA,NSE,CYFRA21-1 and HE4 levels in diagnosis of lung cancer.The levels of CEA and HE4 were higher in patients with adenocarcinoma,the level of CYFRA21-1 was higher in patients with squamous cell carcinoma,the levelof NSE was higher in patients with small cell lung cancer(SCLC)(P<0.05).The detections of CYFRA21-1(AUC=1.000),CEA(AUC=0.727)and HE4(AUC=0.622)in serum are favorable for the diagnosis of squamous cell carcinoma,The detections of serum CEA(AUC=0.954)and HE4(AUC=0.944)levels are favorable forthe diagnosis of adenocarcinoma,and the detections of NSE(AUC=0.876)was favorable for the diagnosis of SCLC(P<0.05).Conclusion The levels of CEA,NSE,CYFRA21-1 and HE4 in serum were abnormal in patientswith lung cancer.The HE4 level in the patients was correlated with the pathological types and the metastasis of lung cancer.The detection of serum HE4 couldbe used in the diagnosis and evaluation of lung cancer.
