摘要:
目的 探讨血液肝素结合蛋白(Heparin-binding protein,HBP)、C反应蛋白(C-reactive protein,CRP)和白细胞计数(White blood cell count,WBC)联合检测对儿童脓毒血症的早期诊断价值。方法 回顾性选取广州市番禺区石碁人民医院2015年2月1日~2017年2月1日收治的儿童脓毒血症患者44例作为脓毒血症组,搜集患儿在开始诊断为脓毒血症的前24 h为脓毒血症组A组,前48 h为脓毒血症B组,前72 h为脓毒血症C组,同期选取43例身体健康的体检者作为对照组。观察比较对照组与脓毒血症A,B和C组HBP,CRP和WBC的指标水平,并采用受试者工作特征曲线(ROC曲线)分析血液HBP,CRP和WBC联合检测对儿童脓毒血症早期诊断价值。结果 脓毒血症A,B和C组的HBP,CRP和WBC指标水平明显高于对照组,脓毒血症A组HBP,CRP和WBC指标水平明显高于脓毒血症B组、C组,各组对比差异有统计学意义(P<0.05)。ROC曲线结果显示,脓毒血症A组HBP,CRP和WBC的最佳临界值分别为17.86 μg/L,16.79 mg/L和0.92×109/L,预测脓毒血症的曲线下面积最大约0.934(0.869~1.002),0.862(0.755~0.967)和0.940(0.872~1.008),各指标比较差异均有统计学意义(P<0.05)。脓毒血症B组HBP,CRP和WBC的最佳临界值分别为13.06 μg/L,15.19 mg/L和0.67×109/L,预测脓毒血症的曲线下面积最大约为0.899(0.813~0.981),0.694(0.540~0.852)和0.819(0.695~0.941),各指标比较差异均有统计学意义(P<0.05)。脓毒血症C组HBP,CRP和WBC的最佳临界值分别为10.32 μg/L,7.89 mg/L和0.29×109/L,预测脓毒血症的曲线下面积最大约为0.822(0.703~0.941),0.719(0.569~0.864)和0.746(0.599~0.889),各指标比较差异均有统计学意义(P<0.05)。结论 血液HBP,CRP和WBC联合检测对儿童脓毒血症早期诊断价值高,其中HBP的窗口期较宽,联合WBC,CRP共同诊断,其准确性、灵敏度更佳,值得临床推广应用。
Abstract:
Abstract:Objective To explore the early diagnostic valueof Heparin-binding protein(HBP),C reactive protein(C-reactive protein,CRP)and leukocyte count(white blood cell count,WBC)in children's sepsis.Methods A retrospective study of 44 children with sepsis in children's Shiji People's Hospital of Panyu District in Guangzhou,from February1st 2015 to February 1st 2017,were used as a sepsis group.The children werecollected in the group A of the pre 24h sepsis group,the anterior 48h was the Bgroup of sepsis,and the former 72h was the C group of sepsis.In the same period,43 healthy persons were selected as control group.The indexes of HBP,CRP and WBC in the control group and the A,B and C groups of the sepsis were compared.The value of the combined detection of HBP,CRP and WBC in blood for the early diagnosis of children's sepsis was analyzed by the working characteristic curve of the subjects(ROC curve).Results The levels of HBP,CRP and WBC in group A,B and C were significantly higher than those in the control group.The level of HBP,CRP and WBC in the A group of sepsis was significantly higher than that in the B group of sepsis,and there was a significant difference between the groups in the C group(P<0.05).The results of ROC curve showed that the best critical values of HBP,CRP and WBC in group A of sepsiswere 17.86 μg/L,16.79 mg/L and 0.92×109/L,and the maximum area under the curve of sepsis was about 0.934(0.869~1.002),0.862(0.755~0.967)and0.940(0.872~1.008)respectively,and all the indexes were statistically significant(P<0.05).The best critical values of HBP,CRP and WBC in B group of sepsis were 13.06 μg/L,15.19 mg/L and 0.67×109/L respectively.Thearea under the curve predicted for sepsis was about 0.899(0.813~0.981),0.694(0.540~0.852)and 0.819(0.695~0.941)respectively and all the indexes were statistically significant(P<0.05).The best critical values of HBP,CRP and WBC in C group of sepsis were 10.32 μg/L,7.89 mg/L and 0.29×109/L respectively.The area under the curve predicted for sepsis was about 0.822(0.703~0.941),0.719(0.569~0.864)and 0.746(0.599~0.889)respectively,and all the indexes were statistically significant(P<0.05).Conclusion The combined detection of blood HBP,CRP and WBC wasof high value in the early diagnosis of children's sepsis.The window period ofHBP was wide,combined with WBC and CRP,and the accuracy and sensitivity are better.It is worthy of clinical application.
