摘要:
目的对粪肠球菌进行多位点序列分析(MLST),探讨浙江地 区粪肠球菌的分子分型与耐药性的相关性。方法收集2018年1~8月 浙江地区多家医院临床分离的粪肠球菌,总计74株;根据粪肠球菌的7个管家基因序列对74 株菌株进行多位点序列(MLST)分型;同时对该74株菌株进行药敏实验,分析MLST与药物耐药 性的相关性。结果74株菌分为20个ST型,3个型别为新的型别。ST1 6和ST179是浙江地区的优势型别菌株,同属于CC16克隆复合体,两者占所有菌株的55%以上 ,其中ST16包含21株菌(占28.38%),其次为ST179,包含20株菌(占27.03%),再次是ST4和 ST6,各包含6株(占8.11%)。药敏结果显示,粪肠球菌耐药程度高的抗生素主要是四环素及 红霉素,其耐药率分别为89.2%和73%,不同ST的粪肠球菌均对这两种抗生素产生耐药。ST1 79与ST16两者耐药谱相似,区别主要是ST179对利福平均为敏感,而ST16有部分(33.3%)会 对利福平产生耐药;另外,ST6对喹诺酮类药物及高浓度的庆大霉素耐药,ST4型的粪肠球 菌会对青霉素产生耐药。结论目前浙江省粪肠球菌分离菌株主要以 CC16(包含ST16与ST179)克隆株为主,不同MLST分型的菌株与耐药可能有一定的相关性,仍 需验证。
Abstract:
Objective To determine the correlation between molecular typing and drug resistance of Enterococcus faecalis from hospitals in Zhejiang by performing multi-locus sequence typing (MLST) analysis on Ent erococcus faecalis .Methods A total of 74 clinical En terococcus faecalis were obtained from hospitals in Zhejiang Province from Jan uary to August 2018.Molecular genotyping was performed by Multi-locus sequence typing (MLST) based on comparison of internal fragment sequences of 7 Enteroc occus faecalis housekeeping gene.Meanwhile,the drug susceptibility tests of the 74 strains were determined according to CLSI standards,and the correlation between STs and drug resistance was analyzed.Results 74 strains were divided into 20 sequence types (ST),including 3 new STs.The domin ant STs in Zhejiang were ST16 and ST179,both belonging to the CC16 clonal compl ex.CC16 accounts for more than 55% of all strains,with ST16 covering 28.38% a nd ST179 covering 27.03%,respectively.The next dominant STs were ST4 and ST6 , each containing 6 strains (8.11%).The drug susceptibility results showed that Enterococcus faecalis was most resistant to tetracycline and erythromycin,a nd the drug-resistant rates were 89.2% and 73%,respectively.Resistance to th ese two drugs were observed in different STs.The resistance spectrums of ST179 and ST16 were similar except that ST179 was all sensitive to rifampin,whilst ST 16 was partly resistant to rifampicin (33.3%).ST6 was found 100% resistant to tetracycline,quinolones and Gentamycin of high concentration.Finally,the only ST that was resistant to penicillin was ST4 (50%).ConclusionAt present,the isolates of Enterococcus faecalis in Zhejiang Province are mainly CC16 (including ST16 and ST179) clones.Different STs strains may ha ve certain correlation with drug resistance,which still needs to be further ver ified.
