目的 探讨血浆血管性血友病因子(von willeBrand factor,vWF),D- 二聚体(D-Dimer,D-D)和外周血 中性粒细胞/ 淋巴细胞比值(NLR)联合检测在血栓性疾病中的临床应用价值。方法 检测52 例血栓性疾病患者和 42 例非血栓性疾病患者的vWF ,D-D 与NLR 水平,采用受试者工作特征(ROC)曲线计算vWF ,D-D 与NLR 单独 及联合检测时,在诊断血栓性疾病中的诊断临界值、曲线下面积(AUC)、敏感度和特异度。结果 血栓性疾病组患 者的血浆vWF,D-D 与NLR 均高于非血栓组,差异均有统计学意义(t =2.988~5.398,均P <0.05)。ROC 曲线分析表 明,vWF ,D-D 与NLR 的诊断临界值分别设定为136.85%,1.14mg/L 和2.857 时,其在诊断血栓性疾病中的ROC 曲 线下面积(AUC)分别为0.783,0.720 和0.766,灵敏度分别为78.85%,63.46% 和73.08%,特异度分别为76.19%, 76.20% 和71.43%。vWF, D-D 与NLR 联合诊断血栓性疾病的AUC 为0.82,灵敏度为82.69%,特异度为71.43%。结论  vWF,D-D 与NLR 升高与血栓性疾病的发生存在明显相关性,联合检测优于各单独指标的应用。
Abstract:
Objective To investigate the diagnostic value of vWF combined with D-Dimer and NLR in serum of patients with thrombotic disease. Methods The results of vWF, D-D and NLR were detected in 52 patients with thrombotic disease (thrombotic disease group) and 42 patients with non-thrombotic disease (non-thrombotic disease group) were detected. The receiver operating characteristic (ROC) curve was used to calculate the diagnostic threshold, area undeer the curve (AUC) diagnostic sensitivity and specificity of vWF, D-D and NLR in the diagnosis of thrombotic diseases when used alone or in combination. Results The sera levels of vWF, D-Dimer and NLR in thrombotic disease group were all higher than those in control group, the difference were statistically significant (t =2.988~5.398,all P <0.05). ROC curve analysis showed that with the critical value of vWF, D-Dimer and NLR in serum were 136.85%, 1.14mg/L and 2.857 respectively. In the diagnosis of thrombotic disease, the area under the ROC curve (AUC) were 0.783,0.720 and 0.766 respectively. The AUC of vWF combined with D-Dimer and NLR in the diagnosis of thrombotic disease was 0.82, the sensitivity and specificity were 82.69% and 71.43% respectively. Conclusion The elevation of vWF, D-D and NLR was significantly correlated with the occurrence of thrombotic diseases, and the combined detection was superior to the application of individual indicators.
