摘要:
目的　探究食道癌 (esophageal cancer, EC)患者血清细胞角蛋白 19片段 (cytokeratin-19-fragment, CYFRA21-1)，糖类抗原 19-9(carbohydrate antigen19-9, CA19-9)和β-人绒毛膜促性腺激素 (β-human chorionic gonadotropin, β-hCG)水平联合检测的诊断价值及与 T淋巴细胞亚群表达的相关性研究。方法　选取 2018年 1月 ~2020年 1月汉中市中心医院收治的 110例 EC患者为 EC组，另选取同期 80例健康志愿者为正常组。采用放射免疫分析法 (radioimmunoassay, RIA)检测 CYFRA21-1的含量，采用酶联免疫吸附法 (enzyme linked immunosorbent assay, ELISA)检测 CA19-9，β-hCG， CD3+，CD4+和 CD8+的水平。比较两组患者 CYFRA21-1，CA19-9，β-hCG，CD3+，CD4+和 CD8+的水平，探究 CYFRA21-1+CA19-9+β-hCG在 EC中的灵敏度和特异度。采用 Spearman相关分析探究 CYFRA21-1+CA19-9+β-hCG与 CD3+，CD4+和 CD8+水平的相关性。结果　EC组患者血清中 CYFRA21-1，CA19-9和β-hCG的含量显著高于正常组，其差异均具有统计学意义 (t=13.283，1.271和 11.261，均 P＜ 0.05)。EC组患者 T淋巴细胞亚群 CD3+，CD4+和 CD8+的水平显著低于正常组，其差异均具有统计学意义 (t=7.520，4.118，4.128，均 P＜ 0.05)。CYFRA21-1诊断 EC的灵敏度和特异度分别为 35.50%和 81.50%，CA19-9诊断 EC的灵敏度和特异度分别为 32.50%和 83.25%，β-hCG诊断 EC的灵敏度和特异度分别为 59.75%和 61.25%，CYFRA21-1+CA19-9+β-hCG诊断 EC的灵敏度和特异度分别为 72.00%和 83.50%，高于单一指标的检测。 CYFRA21-1+CA19-9+β-hCG与 T淋巴细胞亚群 CD3+，CD4+和 CD8+水平呈负相关关系(r=-0.408～0.461，均P＜0.05)。结论　联合检测可有效提高食道癌诊断效能，降低CYFRA21-1，CA19-9和β-hCG水平可改善 EC患者的免疫功能。
