摘要:
目的 探讨外阴鳞癌组织中动力蛋白激活蛋白2(dynactin2,DCTN2) 和同源异型盒基因B7(homeobox geneB7,HOXB7) 水平表达及与预后的价值研究。方法 收集2008 年1 月~ 2017 年12 月在江油市人民医院行手术治疗的132 例外阴鳞癌患者的癌组织与癌旁健康组织,采用免疫组织化学法检测组织中DCTN2 和HOXB7 表达情况;采用Kaplan-Meier 法分析外阴鳞癌患者癌组织DCTN2 和HOXB7 表达与五年生存率之间的关系;采用COX 回归分析外阴鳞癌患者五年后生存情况的影响因素。结果 外阴鳞癌组织中DCTN2 阳性表达低于癌旁健康组织(31.06% vs 71.21%),HOXB7 阳性表达高于癌旁健康组织(65.91% vs 37.12% ),差异具有统计学意义(χ2=42.583,21.899,均P < 0.05)。外阴鳞癌组织DCTN2 和HOXB7 表达与肿瘤FIGO 分期、淋巴结转移、淋巴结转移数量、分化程度和腹股沟淋巴结情况相关(χ2=16.998, 19.144; 6.983, 5.800; 6.595, 7.244; 6.076, 5.665; 5.864, 6.493, 均P < 0.05)。Kaplan-Meier 生存曲线表明DCTN2 阳性表达外阴鳞癌患者生存率高于阴性表达患者(χ2=14.878,P < 0.001),HOXB7 阳性表达患者生存率低于阴性表达患者(χ2=14.824,P < 0.001)。多因素COX 分析结果显示,FIGO 分期、淋巴结转移、淋巴结转移数量、分化程度和HOXB7 表达均是导致外阴鳞癌患者五年死亡的危险因素(P < 0.05),DCTN2 表达为保护因素(P < 0.05)。结论 外阴鳞癌患者癌组织DCTN2 表达下调,HOXB7 表达升高,其表达与患者五年生存预后密切相关。
Abstract:
Objective To explore the expression levels of dynactin2(DCTN2) and homeobox gene B7 (HOXB7) in vulvar squamous cell carcinoma tissue and their prognostic value. Methods The cancer tissues and adjacent healthy tissues of 132 patients with vaginal squamous cell carcinoma who underwent surgical treatment in Jiangyou People’s Hospital from January 2008 to December 2017 were collected, the expression of DCTN2 and HOXB7 was detected by immunohistochemistry. Kaplan- Meier method was applied to analyze the relationship between the expression of DCTN2, HOXB7 and 5-year survival rate in vulvar squamous cell carcinoma. COX regression was applied to analyze the influencing factors of survival of patients with vulvar squamous cell carcinoma after 5 years. Results The positive expression of DCTN2 in vulvar squamous cell carcinoma was lower than that in adjacent healthy tissues (31.06% vs 71.21% ),and the positive expression of HOXB7 was higher than that in adjacent healthy tissues (65.91% vs 37.12% ), the differences were statistically significant (χ2=42.583,21.899,all P<0.05). The expression of DCTN2 and HOXB7 in vulvar squamous cell carcinoma was correlated with tumor FIGO stage, lymph node metastasis, number of lymph node metastasis, degree of differentiation and inguinal lymph nodes (χ2=16.998, 19.144; 6.983, 5.800; 6.595, 7.244; 6.076, 5.665; 5.864, 6.493, all P<0.05). Kaplan-Meier survival curve showed that the survival rate of patients with DCTN2 positive vulvar squamous cell carcinoma was significantly higher than that of patients with negative expression (χ2=14.878,P<0.001), and the survival rate of patients with HOXB7 positive expression was lower than that of patients with negative expression (χ2=14.824,P<0.001). Multivariate COX analysis showed that FIGO stage, lymph node metastasis, number of lymph node metastasis, degree of differentiation, and expression of HOXB7 were all risk factors leading to 5-year death in patients with vulvar squamous cell carcinoma (P<0.05), while expression of DCTN2 was a protective factor (P<0.05). Conclusion The expression of DCTN2 was down-regulated and the expression of HOXB7 was increased in vulvar squamous cell carcinoma, which was closely related to the 5-year survival and prognosis of patients.
