目的 探讨中性粒细胞/淋巴细胞比值(NLR)和血小板计数/淋巴细胞比例(PLR)在原发性胆汁性肝硬化(primary biliary cirrhosis,PBC)患者疾病活动度中的评估价值。方法 回顾性分析2010年1月～2015年7月在上海长海医院及上海长征医院经确诊入院的PBC患者62例,并根据患者Child-pugh评分分为三组,纳入同期健康对照者59例。提取并记录纳入人群的基线资料及实验室指标,并分析NLR和PLR与患者疾病活动度的相关性。对3名PBC患者进行随访,分析治疗前后NLR的变化情况。结果 与健康对照组相比,NLR显著增高,差异有统计学意义(t=3.369,P<0.001),PLR差异无统计学意义(t=0.840,P>0.05)。PBC患者中的NLR水平与TBIL和Mayo评分正相关(R=0.35,0.30,P<0.05),与ALT,AST及ALP的相关性无统计学意义(P>0.05)。对3名PBC患者进行随访,发现临床治疗有效后,NLR水平显著下降,差异有统计学意义(P<0.05)。结论 NLR可能是评估PBC临床疗效的潜在指标。
Abstract:
Objective To explore the clinical significanceof neutrophils/lymphocyte(NLR)and platelet/lymphocyte ratioof(PLR)peripheral blood in patients with primary biliary cirrhosis.Methods Retrospectively analyzed 62 patients with PBC and three subgroup patients according to Child-pugh,who were hospitalized in Shanghai Changhai Hospital and Shanghai Changzheng Hospital from January 2010 to July 2015.Enrolled 59 healthy controls.Baseline data and laboratory indicators were extracted,and the correlation between PLR and NLR and disease activity were analyzed.The changes of NLR in3 patients with PBC were analyzed.3 patients were followed up and their NLRs were recorded before and after therapy.Results Compared with healthy controls,NLR was significantly increased in PBC patients(P<0.05),and not PLR(P>0.05).NLR was positively correlated with TBIL and Mayorisk(R=0.35,0.30,P<0.05),and not ALT,AST or ALP(P>0.05).After therapy,NLRs were significantly reduced in three PBC patients(P<0.05).Conclusion NLR may be a useful biomarker for assessingclinical therapy in PBC patients.
