摘要:
目的 分析目前上海市及周边地区医院实验室对血浆蛋白C活性(PC:A)和蛋白S活性(PS:A)的检测情况,以及临床上对其临床意义的认知情况。方法 回顾性分析2015年1月~2017年12月上海市瑞金医院检验科出凝血实验室对PC:A和PS:A检测情况,收集外单位送检标本(n=20)进行4℃环境保存下不同时间检测PC:A和PS:A,通过EXCEL,Graphpad Prism5.0统计软件进行数据分析。结果 门诊、住院、外单位送检标本及其检测异常率均呈现增长趋势,其中门诊标本量最大,外单位送检量次之,住院标本量最少,2015~2017年度总体异常率分别为4.6%,5.8%和7.8%; 外单位送检共12家医院性质分为:公立医院4家,第三方实验室3家,外资医院2家,民营医院3家,其中外资医院中PC:A和PS:A检测异常率最高,第三方实验室次之; 在4℃环境下不同时间检测结果与即刻相比,PC:A和PS:A在第1,2,3天结果整体呈现平稳下降趋势,差异无统计学意义(P>0.05),PS:A检测结果第3天呈现异常波动,可能与其运输过程中标本保存不当引起污染、变质有关。结论 部分临床医生对血浆蛋白PC和PS的临床意义认知肤浅或缺乏,上海及周边地区医院开展血浆PC和PS活性检测的实验室较少,抗凝蛋白活性检测技术及临床应用必须加强宣传教育和推广普及。
Abstract:
Objective To analyze the detection of plasma protein C activity(PC:A)and protein S activity(PS:A)in hospitals of Shanghaiand surrounding areas,as well as the clinical understanding of its clinical significance.Methods A retrospective investigation was conducted on the detection of plasma PC and PS specimens from the coagulation laboratory of Shanghai Ruijin Hospital from January 2015 to December 2017,and collected other units samples(n=20)and preserved at 4℃ environment,then teststhe plasma activity of PC,PS under different time,and data were analyzed by EXCEL and Graphpad Prism 5.0 statistical software.Results Outpatient,inpatient,and external unit inspection specimens and their abnormal rate of detection showed an increasing trend,among them,the amount of outpatient specimens was the largest.The number of specimens sent by external unitswas second,and the number of hospitalized specimens was the least.The overallabnormal rate were 4.6%,5.8% and 7.8% respectively in 2015~2017.According to the nature of the hospital,there were 12 out-of-bound units,which were divided into four public hospitals,three third-party laboratories,two foreign-funded hospitals and three private hospitals.Among them,the abnormal ratesof PC:A and PS:A in foreign-funded units were the highest,followed by the third-party laboratories.Compared with the immediate test results in 4° environment,PC:A and PS:A showed a steady downward trend on the 1,2 and 3 days,and the difference was not statistically significant(P>0.05).PS:A test results showed abnormal fluctuations on the 3 day,which would be related to the pollution and deterioration caused by improper preservation of specimens during transportation.Conclusion Some clinicians had superficialor insufficient understanding of the clinical significance of plasma protein PCand PS.There were fewer laboratories in hospitals in Shanghai and its surrounding areas to carry out the detection of plasma PC and PS activity.It is necessary to strengthen the popularization and education of anticoagulant protein activity detection technology and its clinical application.
