摘要:
目的 探讨年龄相关性白内障(age related cataract,ARC) 与血清补体 C3,C4 及25 羟基维生素D[25 hydroxyvitaminD,25-(OH)-D] 水平的相关性。方法 选择 2017 年9 月~ 2019 年9 月在咸阳市第一人民医院眼科就诊的134 例ARC 患者和45 例非ARC 患者作为研究对象,依据晶状体混浊程度分为4 组: 非ARC 患者组( 对照组)45 例,早期白内障 (early cataract,EC) 患者组37 例,进展期白内障(progressive cataract,PC) 患者组57 例,成熟白内障(mature cataract,MC) 患者组40 例。应用免疫散射比浊法检测血清补体 C3 和C4 水平,电化学发光分析法检测血清25-(OH)-D 水平,通过视觉质量分析系统测量眼部客观散射指数(objective scattering index,OSI),比较分析以上指标的变化与ARC 的相关性。结果 EC 组、PC 组和MC 组的 OSI 显著高于对照组,且EC 组、PC 组和MC 组的OSI 依次增高,差异均有统计学意义(F=153.712, 均P <0. 01)。EC 组、PC 组和MC 组的 25-(OH)-D,C3 和C4 水平均显著低于对照组,且EC 组、PC 组和MC 组中三项检测指标的水平依次降低,差异均有统计学意义(F=111.623~230.803, 均P <0. 01)。在EC 组、PC组及MC组中,血清25-(OH)-D 分别与C3 和C4 水平具有正相关性(rC3=0.832,813,0.849,P<0.01;rC4=0.798,0.774,0.802,P<0.01)。在EC 组、PC 组及MC 组中,血清 25-(OH)-D,C3 及C4 水平分别与OSI 有负相关性[r25(OH)-D = - 0.783,-0.756,-0.772,均P <0.01;rC3= -0.813,-0.791,-0.809,均P<0.01;rC4= - 0.857,- 0.823,- 0.847,均P <0.01]。结论 补体 C3,C4 和25-(OH)-D 与ARC 的发生、发展有关,调节血清补体 C3,C4 和25-(OH)-D 水平有助于预防和改善ARC 的发生与进展。
