摘要:
目的 探讨外周血血小板参数测定在评价急性脑梗死患者抗栓治疗后生活能力改善的临床价值。方法 收集2016 年10 月~2018 年3 月上海市奉贤区海湾镇五四社区卫生服务中心收治的99 例行抗栓治疗的急性脑梗死患者及54例体检正常者(对照组)的临床资料,在入院及出院时分别对患者进行血小板计数(PLT)、血小板体积(MPV)、血小板分布宽度(PDW)、血小板压积(PCT)和大型血小板比率(PLCR)检测,并在出院时对患者采用改良Rankin评分量表(modified Rankin scale,mRS) 评分,以mRS 评分作为脑卒中终点指标,以mRS 评分≤ 2 分归类为生活能力改善良好组,以mRS 评分≥ 3 分归类为生活能力改善不良组,分别比较入院和出院时生活能力改善良好组与不良组患者的PLT,MPV,PDW,PCT 和PLCR 检验指标差异,并与对照组相比。结果 入院时,生活能力改善良好组与不良组之间PLT,MPV,PDW,PCT 和PLCR 差异均无统计学意义;出院时,生活能力改善良好组的PLT(P=0.008),PCT(P=0.009)显著低于预后不良组。入院时,良好组的PLT(P=0.002),PCT(P=0.007) 显著低于对照组,不良组与对照组比较时差异无统计学意义(均P>0.05);出院时,良好组的PLT(P=0.001),PCT(P=0.001) 显著低于对照组,MPV,PLCR 显著高于对照组,而不良组与对照组比较时差异无统计学意义(P>0.05)。Spearman 相关性分析显示PLT,PCT与患者生活能力改善呈负相关,相关系数分别为-0.241(P=0.008),-0.274(P=0.002)。结论 在评估脑卒中终点生活能力预后时,PLT,PCT 与生活能力改善效果有一定的相关性,但血小板参数在早期预测急性脑梗死患者生活能力改善方面无明显意义,应在急性脑卒中患者抗栓治疗过程中对PLT,PCT 进行密切监测。
