摘要:
目的 探讨外周血白细胞参数中白细胞(white blood Cell,WBC)计数、中性粒细胞(neutrophil,Neu)计数、 淋巴细胞(lymphocyte,Lym)计数以及中性粒细胞与淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)在新型冠 状病毒肺炎(COVID-19)诊断中的临床意义。方法 收集天津市西青医院2019 年10 月~2020 年3 月健康查体人群28 例为健康组,同期该院甲流患者28 例为甲流组,京津冀鲁地区COVID-19 确诊患者28 例为新冠组,均采用流式细胞计 数法进行血细胞检测,使用Graphpad Prism 统计学软件分析三组人群的WBC 计数、Neu 计数、Lym 计数、NLR 值差 异性及COVID-19 患者年龄与NLR 值的相关性。结果 新冠组患者外周血中WBC(q=6.048, P<0.05)计数,Neu(q=5.696, P<0.05)计数及NLR 值(q=2.811, P<0.05)均低于甲流组,差异均具有统计学意义。Lym 计数(q=7.898, P<0.05)低于 健康组而NLR 值(q=4.159, P<0.05)高于健康组,差异具有统计学意义,新冠组中< 50 岁患者与≥ 50 岁患者的NLR 值差异具有统计学意义(t=2.204, P<0.05)。结论 外周血血细胞检测中WBC 计数、Neu 计数、Lym 计数及NLR 值指 标可用于辅助鉴别COVID-19 患者、甲流患者、健康人群,对COVID-19 诊断有一定临床意义,且新冠患者NLR 值具 有年龄差异性。
Abstract:
Objective To investigate the clinical significance of peripheral blood white blood cell (WBC) count, neutrophil (Neu) count, lymphocyte (Lym) count, and neutrophil-to-lymphocyte ratio (NLR) in the diagnosis of COVID-19. Methods A total of 28 health check-up patients were collected as health group from Tianjin Xiqing Hospital from October 2019 to March 2020,and at the same time, 28 cases of patients with influenza A in the hospital were in the group of A-flu, and 28 patients confirmed COVID-19 in Beijing-Tianjin-Hebei-Shandong regions as COVID-19 group. They were given bloodroutine tests performed by flow cytometry. Statistical software Graphpad Prism was used to analyze the differences of WBC count, Neu count, Lym count and NLR value among three groups, and the correlation between age and NLR value of patients with COVID-19. Results The WBC count (q=6.048,P< 0.05), Neu count(q=5.696,P<0.05), and NLR value(q=2.811,P< 0.05) in peripheral blood of COVID-19 group were lower than those in the H1N1 group, and the differences were statistically significant, Lym count(q=7.898,P < 0.05) was lower than that of the healthy group, while NLR value (q =4.159,P< 0.05) was higher than that of the healthy group, and the difference was statistically significant. The NLR values of patients < 50 years old and ≥ 50 years old in the COVID-19 group was significantly different (t=2.204,P< 0.05). Conclusion The WBC count, Neucount, Lym countand NLR valueindexes in peripheral blood routine test can be used to help distinguish COVID-19 patients, H1N1 patients and healthy population, which has certain clinical significance for the diagnosis of COVID-19. Moreover, NLR value of COVID-19 patients has age difference.
