摘要:
目的 研究沙眼衣原体 (CT)及不同基因型导致的生殖道感染与输卵管性不孕( TFI)的相关性,并比较不同 CT基因型宫颈分泌物中衣原体热休克蛋白 60(cHSP60)表达的差异。方法 采用前瞻性队列研究 2018年 1月 ~2019年 6月梅州城区三家医院收集的 254例 TFI和 635例其他原因不孕患者资料和宫颈分泌物标本,采用巢式 PCR 扩增外膜蛋白 Omp1基因的 VD1-VD3序列并测序分型,酶联免疫吸附法 (ELISA) 检测不同 CT基因型感染者宫颈分泌物中 cHSP60表达,分析比较两组患者中 CT基因型的分布情况和生殖道中 cHSP60 表达水平。结果 TFI患者 CT的阳性率(14.6%,37/254)显著高于其他原因不孕患者 CT阳性率( 4.9%,31/635);TFI患者感染最常见的 CT基因型为 E, F和 J型,其他原因不孕患者感染最常见的 CT基因型为 G和 D型,TFI与其他原因不孕患者基因型分布的差异性有统计学意义 (χ2=16.958,P=0.031);CT阳性的 TFI患者生殖道中 cHSP60 表达水平( 111.5±15.0 ng /L)显著高于 CT阳性的其他原因不孕患者 (103.5±9.9 ng /L)(t=2.754,P=0.008);CT阳性的 TFI患者中,E,F和 J基因型感染者生殖道中 cHSP60 表达水平 (116.5±14.9 ng /L)明显高于 D,G基因型 (101.8±9.8 ng /L) (t=3.984,P< 0.001)。结论 不同 CT基因型感染与 TFI可能有关,E,F和 J型可能是与 TFI最密切的基因型。
