摘要:
目的 了解深圳地区妊娠期子痫前期(preeclampsia,PE)孕妇血清中白细胞介素-22(interleukin-22,IL-22)、白细胞介素-17(interleukin-17,IL-17)、白细胞介素-10(interleukin-10,IL-10)、转化生长因子-β(transforming growth factor-β,TGF-β)水平及IL-22基因rs2227485 T/C 位点多态性。方法 选择2019年3月~2020年12月在深圳市龙华区人民医院妇产中心确诊的PE孕妇87例为PE组,120例健康孕妇为对照组。分别检测血清中IL-22,IL-17,IL-10,TGF-β水平及IL-22基因rs2227485 T/C多态性。结果 PE组IL-22,IL-17及TGF-β水平(54.47±16.29ng/L,51.86±15.35ng/L及19.62±2.51ng/ml)明显高于对照组(25.95±6.87ng/L,28.43±6.18ng/L及8.36±1.73ng/ml),差异均有统计学意义(t=6.064 5,4.851 3,8.395 1,均P<0.05),而IL-10水平(22.48±9.01ng/L)明显低于对照组(48.57±15.62ng/L),差异有统计学意义(t=9.106 2,P<0.05)。重度PE孕妇IL-22,IL-17及TGF-β水平(73.86±18.50ng/L,61.70±17.28ng/L及27.06±3.24ng/ml)明显高于轻度PE孕妇(38.72±11.63ng/L,43.86±13.74ng/L及13.57±2.12ng/ml),差异均有统计学意义(t=7.014 5,3.429 0和8.164 7,均P<0.05),而IL-10水平(16.54±8.65ng/L)明显低于轻度PE孕妇(27.31±10.53ng/L),差异有统计学意义(t=4.582 2,P<0.05)。PE组IL-22基因rs2227485 T/C位点CC基因型及C等位基因频率(64.37%和75.29%)明显高于对照组(39.17%和50.83%),差异有统计学意义(χ2=4.621 8,3.497 2,均P<0.05)。重度PE孕妇CC基因型及C等位基因频率(74.36%和83.33%)明显高于轻度PE孕妇(56.25%和68.75%),差异有统计学意义(χ2 =3.736 8,3.014 7,均P<0.05)。CC基因型孕妇IL-22水平(71.05±18.62ng/L)明显高于CT和TT基因型(46.25±11.57ng/L和41.83±10.28ng/L),差异有统计学意义(t=3.506 4,3.723 6,均P<0.05)。PE组孕妇IL-22与IL-17,TGF-β水平呈正相关(r=0.790 3和0.695 8,均P<0.05),而与IL-10水平呈负相关(r=-0.617 2,P=0.031 6)。结论 PE组孕妇IL-22,IL-17及TGF-β水平明显升高,而IL-10水平明显降低,且与病情严重程度有关。同时IL-22基因rs2227485 T/C位点存在多态性,其中CC基因型IL-22水平最高,可能是深圳地区PE孕妇危险易感基因之一。
Abstract:
Objective To investigate the levels of interleukin-22(IL-22) and interleukin-17(IL-17),interleukin-10(IL-10), transforming growth factor-β(TGF-β) and the polymorphism of IL-22 gene rs2227485 T/C locus in pregnant women with preeclampsia (PE) in Shenzhen area. Methods 87 pregnant women with PE who were diagnosed in the Obstetrics and Gynecology Center of Longhua District People’s Hospital of Shenzhen from March 2019 to December 2020 were selected as the PE group and 120 healthy pregnant women as the control group. The levels of IL-22, IL-17, IL-10, TGF-β and the polymorphism of IL-22 gene rs2227485 T/C were detected. Results The levels of IL-22, IL-17 and TGF-β in PE group (54.47±16.29ng/L, 51.86±15.35ng/L and 19.62±2.51ng/ ml) were significantly higher than those in control group (25.95±6.87ng/L, 28.43±6.18ng/L and 8.36±1.73ng/ml), the difference was statistically significant (t=6.064 5, 4.851 3 and 8.395 1, all P<0.05),while the level of IL-10 (22.48±9.01ng/L) was significantly lower than that of the control group (48.57±15.62ng/L), and the difference was statistically significant (t=9.1062, P<0.05). The levels of IL-22, IL-17 and TGF-β in pregnant women with severe PE (73.86±18.50ng/L, 61.70±17.28ng/L and 27.06±3.24ng/ml) were significantly higher than those in pregnant women with mild PE (38.72±11.63ng/L, 43.86±13.74ng/L and 13.57±2.12ng/ml), the difference was statistically significant (t=7.014 5, 3.429 0, 8.164 7, all P<0.05),while the level of IL-10(16.54±8.65ng/L) was significantly lower than that of the pregnant women with mild PE (27.31±10.53ng/L), and the difference was statistically significant (t=4.5822, P <0.05). The CC genotype and C allele frequency (64.37% and 75.29%) of IL-22 rs2227485 T/C locus in PE group were significantly higher than those in control group (39.17% and 50.83%), with statistical significance (χ2=4.621 8, 3.497 2, all P<0.05). CC genotype and C allele frequency (74.36% and 83.33%) in pregnant women with severe PE were significantly higher than those in pregnant women with mild PE (56.25% and 68.75%), the difference was statistically significant (χ2=3.736 8 and 3.014 7, all P<0.05). The level of IL-22 in pregnant women with CC genotype (71.05±18.62ng/L) was significantly higher than that of CT genotype and TT genotype (46.25±11.57ng/L and 41.83±10.28ng/L), and the difference was statistically significant (t=3.5064, 3.7236, all P<0.05). IL-22 was positively correlated with IL-17 and TGF-β(r=0.790 3, 0.695 8, all P<0.05), while negatively correlated with IL-10 (r=-0.617 2, P =0.031 6). Conclusion The levels of IL-22, IL-17 and TGF-β were significantly increased in PE group, while the levels of IL-10 were significantly decreased, which was related to the severity of the disease. At the same time, IL-22 gene rs2227485 T/C site has polymorphism, and the level of IL-22 in pregnant women with CC genotype was the highest, which may be one of the risk susceptibility genes of PE pregnant women in Shenzhen area.
