摘要:
目的 检测子宫肌瘤患者血清和组织中 hsa-miR-15b-5p和 hsa-miR-21-5p水平,并分析其与组织中雌激素受体(estrogen receptor, ER)、孕激素受体( progesterone receptor,PR)蛋白之间的相关性,进而探讨子宫肌瘤发生发展的作用机制。方法 选取 2019年 8月~ 2020年 12月于陆军军医大学士官学校附属医院妇科就诊的 60例子宫肌瘤患者作为研究对象,另取同期因子宫脱垂行切除术的 60例患者作为对照,通过免疫组化 SP法和 Westerblot法检测所有患者肌瘤组织和正常肌层组织中 ER和 PR蛋白的相对表达;术前采所有患者的空腹外周静脉血,通过 qRT-PCR法分别检测患者血清和组织中 hsa-miR-15b-5p和 hsa-miR-21-5p mRNA的相对表达水平,并分析组织中 hsa-miR-15b-5p和 hsa-miR-21-5p水平与 ER,PR蛋白表达的相关性。结果 免疫组化结果显示, ER蛋白在子宫肌瘤组和正常子宫肌层组的阳性表达率分别为 73.3%(44/60)和 38.3%(23/60)(χ2=14.903,P=0.002),PR蛋白在子宫肌瘤组和正常子宫肌层组阳性表达率分别为 65.0%(39/60)和 31.7%(19/60)(χ2=13.348,P=0.005),差异均有统计学意义。 Westernblot结果显示,子宫肌瘤组 ER水平( 0.98±0.21)较正常子宫肌层组( 0.62±0.15)显著升高( t=22.147,P< 0.001),子宫肌瘤组 PR水平( 0.80±0.14)较正常子宫肌层组( 0.38±0.08)亦显著升高( t=38.265,P< 0.001),差异均有统计学意义。 qRT-PCR结果表明,与正常子宫肌层组相比,hsa-miR-15b-5p(1.98±0.12 vs 1.02±0.11)和 hsa-miR-21-5p(2.21±0.15 vs 0.93±0.18)mRNA在子宫肌瘤患者血清中表达水平均显著增加( t=46.080,63.944,均 P< 0.001),在肌瘤组织中两者表达水平较正常肌层组织亦显著升高( t=39.852,52.046,均 P< 0.001),差异均有统计学意义。 Pearson法分析发现,组织中 hsa-miR-15b-5p和 hsa-miR-21-5p表达水平与 ER和 PR蛋白表达水平呈明显正相关( r=0.744~ 0.902,均 P< 0.001)。结论 hsa-miR-15b-5p和 hsa-miR-21-5p在子宫肌瘤患者血清和组织中表达水平均显著升高,子宫肌瘤组织中 ER和 PR蛋白亦呈高表达,并且 hsa-miR-15b-5p和 hsa-miR-21-5p水平与 ER和 PR表达水平呈明显正相关,可能协同诱导子宫肌瘤发生发展。
Abstract:
Objective To detect the expression levels of hsa-miR-15b-5p and hsa-miR-21-5p in serum and tissues of patientswith uterine fibroids, and analyze the correlation between them and ER, PR proteins in tissues, so as to explore the mechanism ofthe occurrence and development of uterine fibroids. Methods From August 2019 to December 2020, 60 patients withhysteromyoma treated in the Department of Gynecology, Affiliated Hospital of Sergeant School of Army Medical University wereselected as the research objects, and another 60 patients who underwent hysterectomy due to uterine prolapse during the sameperiod were selected as the control. The expression of ER and PR protein was detected by SP immunohistochemistry andWesternblot. Peripheral venous blood was collected from all patients on an empty stomach before operation, and the relativeexpression levels of hsa-miR-15b-5p and hsa-miR-21-5p mRNA in the patients’ serum and tissues were detected by qRT-PCRmethod, and analyzed the correlation between hsa-miR-15b-5p and hsa-miR-21-5p mRNA expression levels and ER and PRprotein expression. Results Immunohistochemical results showed that the positive expression rate of ER protein was73.3%(44/60) in leiomyoma group and 38.3%(23/60) in normal myometrium group (χ2 =14.903,P=0.002), and the positiveexpression rates of PR protein was 65.0%(39/60) in leiomyoma group and 31.7%(19/60) in normal myometrium group(χ2=13.348,P=0.005), the differences were statistacally significant respectively. Westernblot results showed that theexpressions of ER in hysteromyoma group (0.98±0.21) compared with the normal myometrium group (0.62±0.15) wassignificantly higher (t=22.147, P < 0. 001), and the PR expression in hysteromyoma group (0.80±0.14) compared with thenormal myometrium group (0.38±0.08) also increased significantly (t=38.265, P < 0. 001), the differences were statistacallysignificant respectively. The qRT-PCR results showed that compared with the normal myometrium group, the expression levels ofhsa-miR-15b-5p(1.98±0.12 vs 1.02±0.11) and hsa-miR-21-5p(2.21±0.15 vs 0.93±0.18) mRNA in the serum of uterinefibroids were significantly increased(t=46.080,63.944,all P < 0.001), and the expression levels of these two proteins inleiomyoma were also significantly higher than those in normal myometriumthe(t=39.852 ,52.046,all P < 0.001), thedifferences were statistacally significant respectively. Pearson method analysis found that the expression levels of hsa-miR-15b-5p and hsa-miR-21-5p in the tissues were significantly positively correlated with the expression levels of ER and PR proteins(r=0.744~0.902,all P < 0.001). Conclusion The expression levels of hsa-miR-15b-5p and hsa-miR-21-5p in serum andtissues of patients with uterine leiomyoma were significantly increased, and ER and PR proteins were also highly expressed inuterine leiomyoma tissues. The expression levels of hsa-miR-15b-5p and hsa-miR-21-5p were positively correlated with ER andPR, which may synergistically induce the occurrence and development of uterine fibroids.
