摘要:
目的 探讨慢性咳嗽变异性哮喘患者外周血单个核细胞信号转导子和转录激活子-1(signal transducer and activator of transcription-1,STAT1)和干扰素调节因子-1(interferon regulatory factor 1,IRF1)蛋白水平与临床急性发作的相关性。方法 收集2018 年8 月~ 2020 年3 月郴州市第一人民医院收治的52 例慢性咳嗽变异性哮喘急性发作患者纳入观察组。同期慢性咳嗽变异性哮喘非急性发作患者52 例纳入对照组。采用蛋白质免疫印迹(western Blot,WB)法检测STAT1 和IRF1 蛋白表达,分析其与慢性咳嗽变异性哮喘急性发作的关系。结果 观察组STAT1(1.28±0.37),IRF1(1.17±0.35)均高于对照组(1.05±0.27, 0.81±0.25),差异有统计学意义(t=3.321,6.036,均P < 0.05);经双变量Pearson 直线相关检验,慢性咳嗽变异性哮喘患者STAT1 与IRF1 蛋白表达呈正相关(r=0.322,P < 0.001);经Logistic 回归分析,STAT1 和IRF1 蛋白过表达可能是诱发慢性咳嗽变异性哮喘急性发作的影响因素(OR > 1,P< 0.001);绘制ROC曲线发现,STAT1,IRF1 蛋白表达及两指标联合评估慢性咳嗽变异性哮喘急性发作的AUC均> 0.85,具有一定预测价值,其中联合检测预测价值最高。结论 慢性咳嗽变异性哮喘急性发作患者STAT1 和IRF1 呈高表达,其水平越高则患者急性发作风险越高,可作为以急性发作预测的有效指标。
Abstract:
Objective To investigate the correlation between the levels of signal transducer and activator of transcription-1 (STAT1), interferon regulatory factor-1 (IRF1) protein in peripheral blood mononuclear cells and clinical acute attacks in patients with chronic cough variant asthma. Methods 52 patients with acute attacks of chronic cough variant asthma treated in the First People’s Hospital of Chenzhou from August 2018 to March 2020 were collected and included in the observation group. 52 patients with non-acute attacks of chronic cough variant asthma during the same period were included in the control group. Western blot (WB) was used to detect the expression of STAT1 and IRF1 protein, and the relationship between them and acute attacks of chronic cough variant asthma was analyzed. Results STAT1 (1.28±0.37), IRF1 (1.17±0.35) of the observation group were higher than the control group(1.05±0.27, 0.81±0.25) , the differences were statistically significant (t=3.321, 6.036, all P<0.05) . The bivariate Pearson linear correlation test showed that the expression of STAT1 and IRF1 protein of patients with chronic cough variant asthma was positively correlated (r=0.322, P<0.001) . The results of Logistic regression analysis showed that the overexpression of STAT1 and IRF1 protein might be the influencing factor for the acute attacks of chronic cough variant asthma (OR>1, P<0.001) .The ROC curve was drawn and found that the AUC of STAT1, IRF1 protein expression and the combination of the two indexes in predicting the acute attacks of chronic cough variant asthma were 0.861, 0.854, both>0.85, which had certain predictive value, of which the combined predictive value was the highest. Conclusion STAT1 and IRF1 in patients with acute attacks of chronic cough variant asthma are overexpressed, the higher the level of them, the higher the risk of acute attacks in patients, which can be used as an effective index to predict acute attack.
