摘要:
目的评价血清清蛋白是否可用于评估系统性红斑狼疮患者疾病活动性。方法2012年6月~2013年12月共有113例患者纳入研究,诊断符合美国风湿病协会要求,男性18例,女性95例,血清清蛋白、补体(C3,C4)、抗dsDNA抗体等检测项目于就诊当天完成,所有患者进行SLEDAI评分,按SLEDAI评分将患者分为活动期组(n=72,SLEDAI≥4)和缓解期组(n=41,SLEDAI=0~4),对两组间上述指标的水平进行比较,同时计算各指标与SLEDAI评分的相关性。结果活动期与缓解期相比,血清清蛋白(g/L),C3(g/L),C4(g/L),抗dsDNA抗体(U/L)水平分别为(35.76±7.19 vs 40.96±7.43,t=3.654,P=0.001),(0.68±0.23 vs 0.90±0.18,t=5.304,P=0.001),(0.13±0.06 vs 0.18±0.05,t=4.007,P=0.001),(229.00±203.95 vs 136.59±82.98,t=-3.384 ,P=0.001),差异均有统计学意义。Pearson相关性分析显示,血清清蛋白,C3,C4,抗dsDNA抗体水平与SLEDAI评分相关性分别为R=0.448,P=0.001;R=0.448,P=0.001;R=0.389,P=0.001和R=0.363,P=0.001。ROC曲线分析显示,血清清蛋白,C3,C4,抗dsDNA抗体在判断狼疮活动与否的曲线下面积分别为0.778,0.783,0.720和0.658,其中血清清蛋白判断狼疮活动与否能力高于抗dsDNA抗体(Z=2.077,P=0.001)。结论血清清蛋白可以作为一个较好的标志物用于对系统性红斑狼疮患者进行疾病活动性评估。
Abstract:
ObjectiveTo evaluate whether serum albumin could evaluate disease activity of patient with systemic lupus erythematosus (SLE).Methods113 SLE patients were consecutively enrolled and allocated into two subgroups according to SLEDAI score,active group (n=72,SLEDAI≥4) and inactive group (n=41,SLEDAI=0~4).All participants been proceeded test for complement 3 (C3),C4,anti-dsDNA antibody,serum albumin and so on.ResultsSerum albumin(g/L),C3(g/L),C4(g/L),anti-dsDNA antibody(U/L) levels between active and inactive group were 35.76±7.19 vs 40.96±7.43,t=3.654,P=0.001;0.68±0.23 vs 0.90±0.18,t=5.304,P=0.001;0.13±0.06 vs 0.18±0.05,t=4.007,P=0.001 and 229.00±203.95 vs 136.59±82.98,t=-3.384,P=0.001,repectively.The correlation between serum albumin,C3,C4,anti-dsDNA antibody and SLEDAI were R=0.448,P=0.001;R=0.448,P=0.001;R=0.389,P=0.001 and R=0.363,P=0.001,repectively.The area under ROC curve of serum albumin,C3,C4,anti-dsDNA antibody in differentiating active from inactive group in SLE were 0.778,0.783,0.720 and 0.658,repectively.It seems that serum albumin was superior to anti-dsDNA antibody in differentiating active from inactive group of SLE (Z=2.077,P=0.001).ConclusionSerum albumin could be considered as an usefull marker for assessment of disease activity in patient with SLE.
