摘要:
目的 探讨血清同型半胱氨酸(Hcy)、胱抑素C(CysC)和尿微量清蛋白(U-mALB)三种生化指标联合检测在糖尿病肾病(DN)早期诊断中的临床价值。方法 根据24 h尿微量清蛋白的定量检测结果将198例2型糖尿病患者分为三组:A组为单纯糖尿病组(n=65),B组为早期糖尿病肾病组(n=68),C组为临床糖尿病肾病组(n=65),分别对这三组患者进行血清Hcy,CysC和24 h U-mALB含量检测,并与50例健康对照组结果进行比较。结果 B组Hcy,CysC和U-mALB水平分别为18.62 ±3.63 μmol/L,2.89±0.54 mg/L和198.33±52.69 mg/24 h; C组分别为23.15±4.16 μmol/L,5.23±1.35 mg/L和383.23±109.52 mg/24 h,均高于对照组和A组,差异有统计学意义(P<0.05); C组Hcy,CysC和U-mALB水平均高于B组,差异有统计学意义(P<0.05); C组和B组的异常率明显高于A组和对照组,差异有统计学意义(P<0.05),且异常率随患者肾损害程度的加重而升高。以单一指标异常作为诊断标准,Hcy,CysC和U-mALB对糖尿病肾损害的异常率分别为72.06%,76.47%和80.88%,三种指标联合检测异常率为95.64%,明显高于单项检测(P<0.05)。结论 联合检测CysC,Hcy和U-mALB三种生化指标对于糖尿病肾病早期诊断、治疗及预后监测具有重要的临床价值。
Abstract:
Objective To research the serum homocysteine(Hcy),Cystatinc(CysC),urine trace albumin(U-mALB)three biochemical indicators combined detection in clinical value for early diagnosis of diabetic nephropathy(DN).Methods According to the quantitative detectionresults of the 24 h urine trace albumin,198 cases of patients with type 2 diabetes was divided into three groups:group A for simple diabetes group(n=65),group B for early diabetic nephropathy group(n=68),and C for clinical diabetic nephropathy group(n=65).For the three groups,the serum Hcy,CysC and 24 h urinary mALB content were detection respectively,and compared with theresults in 50 cases of healthy controls.Results The level of Hcy,CysC and U-mALB in group B and C respectively were 18.62±3.63 μmol/L,2.89±0.54 mg/L,198.33±52.69 mg/24 h and 23.15±4.16 μmol/L,5.23±1.35 mg/L,383.23±109.52 mg/24 h,respectively.The test results of B&C were significantly higher than that of group A(P<0.05).The level of Hcy,CysC and U-mALB in group C were significantly higher than that of group B.The positive detection rate of C and B group was significantly higher than that of A and control group,the difference was statistically significant(P<0.05).The positive rate increased with the process of renal damage degree.Used the single index as diagnostic criteria,the positive rates of Hcy,CysC and U-mALB ondiabetic renal damage were 72.06%,76.47% and 80.88%,respectively,and the positive rate of three indicators of combined detection were significant higher than that of single detection(P<0.05).Conclusion Combined detection of CysC,Hcy,U-mALB three biochemical indicators for early diagnosis,treatment,and prognosis of diabetic nephropathy had important clinicalvalue.
