摘要:
目的 探讨慢性肾病(CKD)患者血清同型半胱氨酸(Hcy)、胱抑素C(Cys C)水平与认知功能障碍的相关性。方法 以2016年1月~2017年5月期间陕西省汉中市中心医院收治的78例CKD患者为研究对象,根据CKD分期分为3期组(n=27)、4期组(n=28)和5期组(n=23)。采用蒙特利尔认知评估量表(MoCA)评价患者的认知水平,据此分为认知功能障碍组(n=26)与正常组(n=52)。比较各组患者的血清Hcy,Cys C,肌酐(SCr)和尿素氮(BUN)水平,分析其与认知水平的相关性。结果①3期组的血清Hcy,Cys C,SCr和BUN水平显著低于另外两组,4期组的Hcy,CysC,SCr和BUN水平均显著低于5期组,差异均有统计学意义(F=19.22166.539,P<0.05)。3期组的MoCA评分显著高于5期组,认知功能障碍发生率显著低于5期组,差异均有统计学意义(F/χ2=3.9936.358,P<0.05)。②认知功能障碍组的血清Hcy,Cys C,SCr和BUN水平均显著高于正常组,差异均有统计学意义(t=2.1983.835,P<0.05)。③Pearson分析结果表明患者的MoCA评分与血清Hcy,Cys C和SCr水平均呈显著负相关(r=-0.370,-0.392,-0.253,P<0.05),与BUN无明显相关性(r=-0.211,P>0.05)。④预测认知功能障碍时,血清Hcy,Cys C,SCr和BUN水平的受试者工作曲线下面积分别为0.818,0853,0.751和0.709(P<0.05)。结论 CKD患者的血清Hcy,Cys水平与认知功能呈显著负相关,其预测价值高于SCr与BUN等传统肾功能指标。
Abstract:
Abstract:Objective To investigate the correlation between serum levels of homocystein(Hcy),Cystatin C(Cys C)and cognitive impairmentin patients with chronic kidney disease(CKD).Methods 78patients with CKD from January 2016 to May 2017 in Hanzhong People's Hospitalin Shaanxi were enrolled for the study and divided into stage 3 groups(n=27),stage 4 group(n=28)and stage 5 group(n=23)according to stage of CKD.Montreal cognitive assessment scale(MoCA)was performed to evaluate the cognitive level of patients,and they were divided into cognitive impairment group and normal group.Serum levels of Hcy,Cys C,creatinine(SCr)and urea nitrogen(BUN)were compared among groups,and the correlation between themand cognitive level was analyzed.Results ①Serum levelsof Hcy,Cys C,SCr and BUN in stage 3 group were significantly lower than that in stage 4 and 5 group,and these indexes in stage 4 group were significantly lower than that in stage 5 group(F=19.221~66.539,all P<0.05).The score of MoCA in stage 3 group was significantly lower than that in stage 5 group,and the incidence of cognitive impairment was significantly higher than that instage 5 group(F/χ2=3.993~6.358,all P<0.05).②The serum levelsof Hcy,Cys C,SCr and BUN in cognitive impairment group were significantly higher than in normal group(t=2.198~3.835,all P<0.05).③Pearson analysis result showed that MoCA score was negatively correlated with serum Hcy,CysC and SCr(r=-0.370,-0.392,-0.253,P<0.05),but there was no significant correlation between score and BUN(r=-0.211,P>0.05).④For prediction of cognitive impairment,area under the receiver operating curve of serum levels of Hcy,Cys C,SCr and BUN were 0.818,0853,0.751 and 0.709(P<0.05).Conclusion Serum levels of Hcy,Cys C were significantly negatively correlated with cognitive function in patients with CKD,whose predictive value were higher than that of traditional renal function indexes such as SCr and BUN.
