摘要:
目的 探讨血清同型半胱氨酸 (homocysteine, HCY) 、叶酸(folic acid, FA) 和维生素B12(vitamin B12,Vit B12) 水平与老年脑白质疏松症(leukoaraiosis,LA) 的相关性。 方法 将2016 年6 月~2019 年1 月神经内科住院的患者111 例纳入研究,检测患者血清 HCY,FA 和Vit B12 水平。应用磁共振成像(magnetic rasonance imaging,MRI) 技术,对患 者行头部磁共振扫描,根据LA 是否存在,分为LA 组及无LA 组,根据LA 严重程度,分为轻度组、中度组和重度组。 结果 LA 组的HCY 水平明显高于无LA 组,LA 组的FA 和Vit B12 水平明显低于无LA 组,两组比较的差异均具有统 计学意义(t =5.538,3.109,4.659,均P <0.01)。中度组和重度组的HCY 水平明显高于轻度组,中度组的HCY 水平明显 低于重度组,差异均具有统计学意义(t =2.712~5.505,均P <0.01 ),轻度组的FA 水平明显高于中度组及重度组,差异 均具有统计学意义(t =2.238,2.099,均P <0.05 ) ,轻度组的Vit B12 水平明显高于中度组及重度组,差异均具有统计学 意义(t =2.433,2.966,均P <0.05)。血清HCY 水平与LA 程度呈正相关(r=0.344,P <0.01 ),血清Vit B12 水平与LA 程 度呈负相关 (r = - 0.286,P <0.05 )。 结论 HCY,FA 和Vit B12 水平与LA 具有密切的相关性。对老年患者行HCY, FA 和Vit B12 检测具有重要的临床价值。
Abstract:
Objective To investigate the correlation between serum (homocysteine HCY), folate acid (FA), vitamin B12 (Vit B12)and leukoaraiosis (LA) in elderly patients. Methods 111 patients were enrolled in the study in the department of neurology from June 2016 to January 2019. The serum levels of HCY, FA and Vit B12 were detected. Magnetic rasonance imaging (MRI) was uesd for patients with head scanning. According to whether the presence of leukoaraiosis, patients were classified as LA group and no LA group. According to the degree of LA, patients were classified as mild group, moderate group and severe group. Results The HCY levels in the LA group were significantly higher than no LA group, while the levels of FA and Vit B12 were significantly lower than no LA group, the differences were statistically significant (t =5.538, 3.109, 4.659, all P <0.01). The HCY levels in moderate group and severe group were significantly higher than mild group, and the HCY levels in severe group were significantly higher than moderate group, the differences were statistically significant (t =2.712~5.505, all P <0.01). The FA levels in moderate stenosis group and severe group were significantly lower than mild group, the differences were statistically significant (t =2.238, 2.099, all P <0.05). The Vit B12 levels in moderate group and severe group were significantly lower than mild group, the differences were statistically significant (t =2.433, 2.966, all P <0.05). The degree of LA and serum levels of HCY were positively correlated (r =0.344, P <0.01). The degree of LA and levels of Vit B12 were negatively correlated (r = - 0.286, P <0.05). Conclusion The levels of HCY, FA and Vit B12 were closely related to LA. HCY, FA and Vit B12 play important roles in elderly patients.
