摘要:
目的　研究高级别浆液性卵巢癌(high-grade serous ovarian cancer, HGSOC)患者血清miR-1246和miR-2278检测水平的临床诊断价值。方法　选取西安高新医院妇科收治的55例HGSOC患者为试验组，另选取同期51例体检健康者为对照组，采用实时荧光定量PCR (rt-qPCR)检测血清中miR-1246和miR-2278的水平，用受试者工作特征曲线(Receiver operating characteristic curve, ROC)分析血清中糖类抗原125(carbohydrate antigen 125, CA125)，miR-1246和miR-2278单独和联合检测的诊断效能。结果　试验组患者的CA125，miR-1246和miR-2278水平均显著高于对照组，差异具有统计学意义(t=7.39~9.35，均P＜0.05)。ROC曲线分析显示，CA125，miR-1246和miR-2278联合检测的敏感度和特异度分别为98.7%和79.5%，高于单一检测指标。血清miR-1246和miR-2278上调在FIGO分期、有无淋巴结转移和有无复发比较差异无统计学意义(χ2=1.07~1.97，均P＞0.05)，血清miR-1246和miR-2278上调在远处转移患者中的比例显著高于无远处转移患者，差异具有统计学意义(χ2=12.31，P=0.00)。结论　血清miR-1246和miR-2278联合CA125水平检测对诊断HGSOC有潜在的应用价值。
Abstract:
Objective　To investigate the clinical diagnostic value of serum levels of miR-1246 and miR-2278 in patients withhigh-grade serous ovarian cancer (HGSOC). Methods　55 HGSOC patients admitted to the department of gynecology ofXi’an Gaoxin Hospital were selected as the test group. In addition, 51 healthy patients in physical examination during the sameperiod were selected as the control group. Real-time quantitative PCR (rt-qPCR) was used to detect the absolute concentrationof miR-1246 and miR-2278 in serum. Receiver operating characteristic (ROC) curve was used to analyze the diagnostic efficacyof HGSOC in serum carbohydrate 125 (CA125), and miR-1246 and miR-2278 separately and combined to detect HGSOC.Results　The levels of CA125, miR-1246 and miR-2278 in experimental group were significantly higher than those in controlgroup, and the difference was statistically significant (t=7.39~9.35, all P<0.05). ROC curve analysis showed that the sensitivityand specificity of CA125, miR-1246 and miR-2278 combined detection reached 98.7% and 79.5%, higher than a single detectionindex. There was no statistically significant difference in the upregulation of serum miR-1246 and miR-2278 in FIGO stage,lymph node metastasis or recurrence (χ2=1.07~1.97, all P>0.05). The proportion of serum miR-1246 and miR-2278 upregulationin patients with distant metastasis was significantly higher than that in patients without distant metastasis, and the difference wasstatistically significant (χ2=12.31, P=0.00). Conclusion　Combined CA125 level detection of serum miR-1246 and miR-2278has potential application value in the diagnosis of HGSOC.
