摘要:
目的 探讨尿液10 项生化指标对高血压肾病(HTN)的比较分析与优选出的尿液5 项生化指标对HTN 的诊断价值。方法 测定43 例HTN 患者、52 例高血压患者和55 例健康体检者的尿液10 项生化指标,并运用IBM SPSS23.0 与SAS 9.4 统计软件对结果进行统计分析。结果 HTN 患者的尿液α1- 微球蛋白(α1-MG),胱抑素C(Cys-C),尿视黄醇结合蛋白(RBP),肾损伤分子-1(Kim-1),N- 乙酰-β-D- 氨基葡萄糖苷酶(NAG),尿转铁蛋白(TRF),中性粒细胞明胶酶相关脂质运载蛋白(NGAL),β2- 微球蛋白(β2-MG),免疫球蛋白G(IgG)和尿微量清蛋白(mALB)结果都明显高于高血压患者和健康体检者( 均P =0.000)。典型相关分析选出的尿液α1-MG,RBP,Kim-1,TRF 和NGAL,对HTN 的诊断效能最大;它们的ROC 曲线下面积分别为0.984 8,0.986 6,0.959 5,0.988 2 和0.991 7。用尿液的α1-MG ≥ 11.6 mg/L,RBP ≥ 1.175 mg/L,Kim-1 ≥ 0.89μg/L,TRF ≥ 3.15 mg/L 和NGAL ≥ 98.5μg/L 的5项之2 诊断HTN,敏感度、特异度和诊断符合率分别为97.67%,96.83% 和97.17%。结论 联合检测高血压患者的尿液α1-MG,RBP,Kim-1,TRF 和NGAL,能早期诊断HTN。
