摘要:
目的 探讨尿白细胞介素 -17(IL-17)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)与高迁移率族蛋白 B1(HMGB1)水平在狼疮性肾炎(LN)中的变化及其临床意义。方法 选取三亚中心医院收治的 LN 患者 106 例,即Ⅱ型,Ⅲ型,Ⅳ型,Ⅴ型和Ⅵ型分别为 18,17,43,21 和 7 例。采用 SLE 病情活动指数(SLEDAI)评分将 106 例 LN 患者分为 LN 稳定组(n=37,SLEDAI 评分< 10 分)和 LN 活动组(n=69,SLEDAI 评分≥ 10 分),采用酶联免疫吸附法测定各组尿 IL-17,NGAL 及 HMGB1 水平。应用 ROC 曲线分析尿 IL-17,NGAL 及 HMGB1 水平预测活动性 LN 的价值。结 果 LN 组 尿 IL-17(146.30±38.24ng/L vs 48.70±11.25ng/L),NGAL(230.72±65.20ng/ml vs 27.36±6.40ng/ml)和 HMGB1(21.73±6.25μg/L vs 3.46±1.20μg/L)水平均明显高于对照组,差异均有统计学意义(t=15.826,18.847,14.715,均 P<0.001)。LN 活动组尿 IL-17(171.42±45.26ng/L vs 120.28±26.70ng/L),NGAL(257.63±69.24ng/ml vs196.35±47.38ng/ml)和 HMGB1(27.16±7.83μg/L vs 16.15±4.17μg/L)水平均明显高于 LN 稳定组,差异均有统计学意义(t=10.264,13.718,9.516,均 P<0.001)。不同分型 LN 患者尿 IL-17,NGAL 及 HMGB1 水平均明显高于对照组(F=8.615,12.217,8.295,P<0.001),且Ⅴ~Ⅵ型组和Ⅳ型组尿 IL-17,NGAL 及 HMGB1 水平均明显高于Ⅱ~Ⅲ型组(P<0.001),差异有统计学意义。ROC曲线显示,IL-17,NGAL及HMGB1三项联合预测活动性LN的曲线下面积(0.918,95%CI:0.856~0.982)最大,其敏感度(92.6%)和特异度(86.5%)较好。结论 LN 患者尿 IL-17,NGAL 及 HMGB1水平明显升高,且与 LN 活动性及病理分型相关,三项联合预测活动性 LN 具有较高的价值。
