摘要:
目的 探讨缺血性脑血管病患者血清同型半胱氨酸(homocy steine,HCY)与颈动脉狭窄及斑块稳定性的相关性。方法 将2016年 6月~12月入住第四军医大学唐都医院的缺血性脑血管病患者154例纳入研究,检测患者血清 HCY水平。应用CT血管成像(CT angiography,CTA)技术,对患者行颈部CTA扫描。根据血清H CY 水平差异,将其分为高HCY组(80例观察组)和正常HCY组(74例对照组),比较两组患 者颈动脉狭窄程度、斑块数量及稳定性,并比较不同颈动脉狭窄程度及斑块稳定程度组间HC Y水平的差异。结果 观察组血管总狭窄率显著高于对照组,观察组 中、重度狭窄率显著高于对照组,差异均具有统计学意义(χ2=22.506,5.980,5.5 94,均P<0.05)。轻、中、重度狭窄组血清HCY水平分别为13.16±6.73,15.19± 5.93和26.13±11.18 μmol/L,中、重度狭窄组血清HCY水平显著高于轻度狭 窄组,重度狭窄组血清HCY水平显著高于中度狭窄组,差异均具有统计学意义(t=3.054 ,5.270,2.684,均P<0.01)。观察组颈动脉斑块检出率显著高于对照组,差异具有统 计学意义(χ2=25.053,P<0.01)。观察组不稳定斑块及混合斑块检出率显著高于对 照组,稳定斑块检出率显著低于对照组,差异均具有统计学意义(χ2=4.870,4.067 ,14.95,均P<0.05)。稳定斑块组、混合斑块组及不稳定斑块组血清HCY水平分别为16 .14±5.49,21.91±6.32和26.74±10.59 μmol/L,混合斑块组 及不稳定斑块组血清HCY水平显著高于稳定斑块组,差异均具有统计学意义(t=4.370,4 .628,均P<0.01),不稳定斑块组血清HCY水平显著高于混合斑块组,差异具有统计学意 义(t=2.249,P<0.05)。结论 血清HCY水平与颈动脉狭窄及斑 块稳定性具有密切相关性,高HCY血症可使颈动脉狭窄发生率及狭窄程度增高,颈动脉斑块 总数及不稳定斑块数量增多。
Abstract:
Objective To investigate the correlation betwee n serum HCY (Homocysteine) and carotid artery stenosis,plaque stability in pati ents with ischemic cerebrovascular disease.Methods 154 p atients with ischemic cerebrovascular disease in Tangdu Hospital were enrolled i n the study from June to December 2016.The serum levels of HCY were detected .CT angiography (CTA) was uesd for patients with neck vascular scanning.Acco rding to the difference of serum HCY level,patients were divided into 80 cases of high HCY group (observation group) and 74 cases of normal HCY group (control group).The degree of carotid artery stenosis,number and stability of plaque we re compared between the two groups and the correlation between serum HCY level a nd degree of carotid artery stenosis and plaque stability were analyzed.〖WT5” HZ〗Results The total stenosis rate in the observation group was sig nificantly higher than that in the control group,the moderate stenosis rate and severe stenosis rate in the observation group were significantly higher than th ose in the control group,with the statistically significant differences (χ 2=5.594~22.506,all P<0.05).The levels of serum HCY in mild,moderate and severe stenosis group were 13.16±6.73,15.19±5.93 and 26.13±11.18  μmol/L respectively.The levels of HCY in moderate stenosis group and severe stenosis group were significantly higher than that in mild stenosis group,and t he levels of HCY in severe stenosis group was significantly higher than that in moderate stenosis group,with the statistically significant differences (t=2 .684~5.270,all P<0.01).The rate of carotid plaque in the observation g roup was significantly higher than that in the control group,and the difference s statistically significant (χ2=25.053,P<0.01).The rate of unstable p laque and mixed plaque in the observation group was significantly higher than th at in the control group,and the rate of stable plaque was significantly lower t han that in the control group (χ2=4.067~14.95,all P<0.05).The l evels of serum HCY in stable plaque group,mixed plaque group and unstable plaqu e group were 16.14±5.49,21.91±6.32 and 26.74±10.59 μmol/L respective ly.The levels of HCY in mixed plaque group and unstable plaque group were signi ficantly higher than that in stable plaque group,and the differences were stati stically significant (t=4.370,4.628,all P<0.01).The level of HCY i n unstable plaque group was significantly higher than that in mixed plaque group ,and the difference was statistically significant (t=2.249,P<0.05).〖WT 5”HZ〗Conclusion Serum HCY levels were closely related to carotid a rtery stenosis and plaque stability.Hyperhomocysteinemia can increase the incid ence and degree of carotid artery stenosis as well as the number of carotid plaq ues and unstable plaques.
