摘要:
目的 探讨妊娠期高血糖患者血浆同型半胱氨酸(homocysteine,Hcy)与脐动脉血流参数改变的相关性。方法 收集2017年1月~2018年6月于汉中市中心医院产科住院的孕妇共210例,其中正常晚期孕妇(NC组)75例,妊娠期糖尿病孕妇(GDM组)65例,孕前糖尿病孕妇(PGDM组)70例,分别检测并比较各组孕妇血清Hcy水平及脐动脉血流[收缩末期最大血流速度与舒张期最大血流速度之比(S/D),阻力指数(RI)及搏动指数(PI)],并分析脐动脉血流与血浆Hcy水平的相关性。结果 三组孕妇临床资料比较差异无统计学意义(F=0.559~0.950,P>0.05)。PGDM组、GDM组与NC组血清Hcy水平分别为10.33±1.78,6.78±1.31和3.89±1.21 μmol/L,各组之间比较差异具有统计学意义(F=27.66,P=0.000)。PGDM组、GDM组与NC组S/D分别为3.91±0.75,3.22±0.64和2.71±0.46; RI分别为0.80±0.06,0.63±0.06和0.51±0.05; PI分别为1.42±0.71,1.12±0.29和0.65±0.33,各组之间差异均具有统计学意义(F=6.765,4.398,7.890; P=0.005,0.013,0.002)。PGDM组孕妇血清Hcy与脐动脉血流参数S/D,RI,PI呈正相关(r=0.801,0.719,0.459; P=0.001,0.002,0.021)。GDM组血清Hcy与脐动脉血流参数S/D,RI,PI呈正相关(r=0.559,0.592,0.311; P=0.008,0.007,0.002)。结论 Hcy在孕期高血糖患者(包括妊娠期糖尿病及孕前糖尿病)中明显升高,孕前糖尿病患者S/D水平明显升高。降低Hcy、监测S/D可能是治疗妊娠期高血糖的措施之一。
Abstract:
Objective To analyze the relationship between the levels of serum total homocysteine(Hcy)with the change in umbilical arterial in the gestational diabetes mellitus.Methods Totally 75 normal late pregnancy(NC group),65 gestational diabetes mellitus pregnancies(GDM group)and 70 pregestational diabetes mellitus(PGDM group)in Hanzhong Central Hospital were recruicted in this study from January 2017 to June 2018.Thelevel of serum homocysteine and the umbilical artery blood flow parameter weredetected.Correlation tests were conducted between these indicators.Results There was no significant difference in the clinical data between the PGDM,GDM and NC group(F=0.559~0.950,all P>0.05).The Hcy expression levels of PGDM,GDM and NC groups were 10.33±1.78,6.78±1.31and 3.89±1.21 μmol/L,respectively.The difference between the groups was statistically significant(F=27.66,P=0.000).The S/D of PGDM,GDM and NC groups were 3.91±0.75,3.22±0.64 and 2.71±0.46,RI were 0.80±0.06,0.63±0.06 and 0.51±0.05,PI were 1.42±0.71,1.12±0.29 and 0.65±0.33,respectively.The difference between the groups were statistically significant(F=6.765,4.398,7.890,P=0.005,0.013,0.002).Hcy of PGDM group was positively correlated with S/D,RI and PI(r=0.801,0.719,0.459,P=0.001,0.002,0.021).And Hcy of GDM group was positively correlated with S/D,RI and PI(r=0.559,0.592,0.311,P=0.008,0.007,0.002).Conclusion The levels of Hcy and S/D were significantly higher with HIP(gestational diabetes mellitus and pregestational diabetes mellitus),andS/D was significantly higher with PGDM.To reduce Hcy level and monitor S/D would be new ways for the treatment of HIP.
