目的 评估标本溶血对固相免疫层析法检测降钙素原(procalcition,PCT)结果的影响。方法 收集2016年11月无溶血标本共66例,其中PCT<0.5 ng/ml组为22例,0.5 ng/ml≤PCT<10 ng/ml组为22例,PCT≥10 ng/ml组22例。将66例无溶血标本制备成不同血红蛋白浓度(0,3,6,9,12,15 g/L)的样本,离心后取50 μl血浆,采用固相免疫层析法检测PCT值,计算经稀释因子校正后PCT值,分析不同溶血程度的标本对PCT结果的影响。结果 0.5 ng/ml≤PCT<10 ng/ml组中,当血红蛋白浓度为12,15 g/L时,PCT值与对照组相比差异有统计学意义(t=6.71,P=0.000 1; t=6.91,P=0.000 0),PCT值是溶血前PCT值的65%和56%。PCT≥10ng/ml组中,当血红蛋白浓度为9,12,15 g/L时,PCT值与对照组相比差异有统计学意义(t=4.04,P=0.000 2; t=6.91,P=0.000 0; t=4.04,P=0.000 2),PCT值是溶血前PCT值的0.73%,0.61%和0.53%。结论 溶血对PCT检测有干扰作用,使得PCT值假性偏低。医务人员应做好分析前质量控制,尽量避免标本溶血。
Abstract:
Abstract:Objective To explore the effect of clinical hemolytic specimens on the detection of procalcitonin by solid phase immunochromatography.Methods 66 cases of samples without hemolysis were collected in Nov 2016,included 22 cases in group PCT<0.5 ng/ml,22 cases ingroup 0.5 ng/ml ≤PCT<10 ng/ml and 22 cases in group PCT≥10 ng/ml.66 cases ofsamples without hemolysis were prepared into samples with different hemoglobinconcentration(0,3,6,9,12,15 g/L).After centrifugation,50 μl plasma wastaken and PCT value was detected by solid phaseimmunochromatography assay.PCT value corrected by dilution factor was calculated.The effect of different hemolysis degree on PCT results was analyzed.Results In 0.5 ng/ml≤PCT<10 ng/ml group,when the concentration of hemoglobin was 12,15 g/L,PCT value had significant difference compared with the control group(t=6.71,P=0.000 1; t=6.91,P=0.000 0).PCT value was 65% and 56% compared before hemolysis.In PCT≥10 ng/ml group,when the concentration of hemoglobin was9,12 and 15 g/L,PCT value had significant difference compared with the controlgroup(t=4.04,P=0.000 2; t=6.91,P=0.000 0; t=4.04,P=0.000 2).PCT value was 73%,61% and 53% compared before hemolysis.ConclusionHemolysis had a disturbing effect on PCT detection,making PCT false low.The medical staff should do a good job of quality control before analysis,and try to avoid the hemolysis of the specimens.
