摘要:
目的 探讨检测纤维蛋白单体对诊断血液透析患者是否发生冠心病的临床意义。方法 血液透析患者依据是否发生冠心病而分为两组,血液透析患者发生冠心病组依据冠脉造影分为急性心肌梗死(AMI) 组28 例,稳定型心绞痛(SAP)组20 例和不稳定型心绞痛(UAP) 组12 例。对所有血液透析患者及正常对照组进行血浆凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原降解产物(FDP)、D- 二聚体(D-D)和纤维蛋白单体(FM)定量检测并进行统计学分析。结果 正常对照组分别与血液透析患者有无冠心病组的TT,PT,APTT 和FDP 水平比较差异无统计学意义(t=0.14 ~ 0.48,均P ﹥ 0.05)。而D-D 和FM 指标相比差异有统计学意义(t=4.37 ~ 8.68,均P ﹤ 0.05)。血液透析患者有无冠心病组间TT,PT,APTT,FDP 和D-D 水平比较差异均无统计学意义(t=0.37 ~ 0.58,均P ﹥ 0.05)。而FM指标比较差异有统计学意义(t=12.25,P ﹤ 0.05)。血液透析患者AMI 组比SAP 和UAP 组的D-D 和FM水平均增高,差异有统计学意义(t=4.89 ~ 13.75,均P ﹤ 0.01)。SAP 和UAP 组D-D 水平差异无统计学意义(t=0.38,P ﹥ 0.05)。而纤维蛋白单体水平差别有统计学意义(t=14.06,P ﹤ 0.01)。结论 FM 是血液透析患者并发冠心病诊断的敏感指标。该指标可界定稳定型和不稳定型心绞痛。
