摘要:
目的 探讨 C-反应蛋白(C-reactive protein,CRP)与清蛋白(albumin,ALB)比值(C-reactive protein to albumin ratio,CAR)和红细胞分布宽度(red blood cell distribution width ,RDW)在强直性脊柱炎(ankylosing spondylitis ,AS)诊断中的应用价值。方法 选取西安市第五医院 2019 年 1 月~ 2020 年 11 月收治的 107 例 AS 患者和 100 例体检健康者作为研究对象。检测研究对象的 RDW,红细胞沉降率(erythrocyte sedimentation rate ,ESR)、CRP 和 ALB 并计算 CAR,比较两组间 CAR 和 RDW 的差异,分析各检测指标间的相关性,受试者工作特征(ROC)曲线评价 CAR 和 RDW 单独及联合检测诊断 AS 的价值。结果 AS 组 RDW,ESR,CRP,CAR 与健康对照组比较,差异均有统计学意义(Z= -11.30 ~ -7.77, 均P<0.001)。相关性分析显示 CAR 与 CRP,ESR 呈正相关(r=0.994,P<0.001;r =0.681,P<0.001);RDW 与 CRP,ESR 呈正相关(r=0.235,P<0.001;r =0.262,P<0.001)。CAR 诊断 AS 的曲线下面积(AUC)为 0.946;RDW 诊断 AS 的 AUC 为 0.814。联合诊断试验结果表明,CAR 与 RDW 并联试验(CAR/RDW)诊断 AS 的敏感度和特异度分别为97.20% 和 81.00%,CAR 与 RDW 串联试验(CAR+RDW)诊断 AS 的敏感度和特异度分别为57.94% 和 100.00%。结论 CAR 和 RDW 对 AS 的诊断具有一定临床价值,CAR 和 RDW 联合检测可提高诊断 AS 的敏感度和特异度。
Abstract:
Objective To explore the diagnostic value of C-reactive protein to albumin ratio(CAR) and red blood cell distribution width(RDW)in ankylosing spondylitis(AS). Methods A total of 107 AS patients and another 100 healthy people admitted to Xi’an Fifth Hospital were selected from January 2019 to November 2020. RDW, erythrocyte sedimentation rate (ESR),C-reactive protein (CRP) and albumin (ALB) levels were detected, CAR was calculated. The differences of CAR and RDW between the two groups were compared. The correlation between the laboratory indicators were analyzed by correlation analysis. The clinical significance of CAR and RDW in the diagnosis of AS was investigated by receiver operating characteristic (ROC) curve, and combined diagnostic test was performed. Results The levels of RDW, ESR, CRP, CAR in AS group were significantly different from those in healthy control group (Z= -11.30 ~ -7.77, all P < 0.001). The correlation analysis showed that CAR was positively correlated with CRP and ESR (r=0.994,P<0.001;r=0.681,P<0.001), RDW was positively correlated with CRP and ESR (r=0.235,P<0.001;r=0.262,P<0.001). The area under the curve (AUC) of CAR was 0.946, the AUC of RDW was 0.946. The sensitivity and specificity of CAR and RDW parallel testing (CAR/RDW) were 97.20% and 81.00%, and the sensitivity and specificity of the CAR and RDW tandem testing (CAR+RDW) were 57.94% and 100.00%, respectively. Conclusion CAR and RDW both have clinical value for the diagnosis of ankylosing spondylitis, and the combination of the two can improve the diagnostic sensitivity and specificity.
