摘要:
目的　探讨急性心力衰竭（acute heart failure, AHF）心功能Ⅳ级患者血清同型半胱氨酸（Hcy）水平与性别、年龄、N- 末端脑钠肽前体（NT-ProBNP），脂蛋白（a）[LP（a）] 及住院时间的相关性。方法　选取 2017 年 1 月 ~2019年 6 月收治的急性心力衰竭患者 32 例为研究对象，其中包括女性 22 例、男性 10 例，年龄 58~88 岁，平均 74.47±7.35岁。所有研究对象行血清 Hcy，NT-ProBNP 和 LP（a）检测。分析比较急性心力衰竭心功能Ⅳ级患者血清 Hcy 与性别、年龄、NT-ProBNP，LP（a）及住院时间的相关性。结果　男性与女性 Hcy，NT-ProBNP，LP（a）和住院时间分别为 19.87±9.08μmol/L vs 18.10±8.95μmol/L、13 930.6±10 863.2pg/ml vs 7 935.3±7 663.7pg/ml、249.8±254.2mg/L vs332.9±269.4mg/L 和 7.13±2.57 天 vs 8.56±3.06 天，除 Hcy 外差异无统计学意义（t=2.18，P>0.05）外其余差异均有统计学意义（t=4.51，3.06 和 2.37，P 均 <0.05）。根据 Spearson 相关分析提示血清 Hcy 与性别、年龄和 LP（a）均不存在相关性，与男性 NT-ProBNP、女性住院时间存在相关性，相关系数为 0.679 和 0.416。结论　心衰患者血清 Hcy 异常可以反映患者心衰严重程度。
