摘要:
目的 探讨2 型糖尿病患者血清低清蛋白水平对糖化清蛋白检测结果的影响。方法 回顾性统计分析了82 例2 型糖尿病患者的实验室检查结果, 按照实验室血清清蛋白< 35 g/L 为异常,其中血清清蛋白正常患者41 例为对照组,血清低清蛋白患者41 例为观察组;分别记录清蛋白(albumin,ALB)、糖化清蛋白(glycosylated albumin,GA)、空腹血糖(FBG)和糖化血红蛋白(HbA1c),并分别计算GA/ALB 比值和GA/HbA1c 比值,然后统计比较两组患者的ALB,GA,FBG,HbA1c,GA/ALB 和GA/HbA1c,并分析GA 与ALB,FBG,HbA1c,GA/ALB 和GA/HbA1c 的相关性。结果 与对照组比较,FBG,HbA1c 和GA/ALB 差异均无统计学意义(t=0.087,0.074,0.924, 均P>0.05), 而ALB,GA 和GA/HbA1c 均显著降低,差异均有统计学意义(t=11.463,3.588,4.186,均P<0.05)。在对照组和观察组中GA 与ALB的相关性均无统计学意义(r=-0.238,P=0.183;r=0.383,P=0.053);在对照组中,GA 与FBG,HbA1c 均有强相关性,差异均有统计学意义(r=0.758,P=0.000;r=0.884,P=0.000);但是在观察组中GA 与FBG,HbA1c 相关性差异均无统计学意义(r=0.157,P=0.444;r=0.309,P=0.125)。结论 在2 型糖尿病患者伴有血清低ALB 时,检测的GA 结果存在假性降低,故在使用GA 监测2 型糖尿病患者短期血糖控制情况时, 需重视此影响。
